二维超声检查与ABUS在乳腺微小结节良恶性鉴别中的诊断效能

摘要 目的:探究二维超声检查与自动乳腺超声诊断系统(ABUS)在乳腺微小结节良恶性鉴别中的诊断价值。方法:选择本院2018年10月-2020年2月收治的经病理检查明确为乳腺微小结节患者80例(111个结节),所有患者均行二维超声及ABUS检查。比较不同检查方法在各象限中乳腺微小结节的检出情况,分析二维超声、ABUS在乳腺微小结节良恶性诊断中的应用价值,另分析二维超声及ABUS检查与病理检查结果的一致性。结果:ABUS乳腺微小结节检出率均高于二维超声,差异有统计学意义(P<0.05);ABUS鉴别诊断乳腺微小结节良恶性的灵敏度、特异度、准确度均高于二维超声,差异有统计学意义(P<0.05)。Kappa检验显示:ABUS与病理结果的一致性良好(Kappa=0.891,P=0.000);二维超声与病理结果的一致性不佳(Kappa=0.396,P=0.000)。结论:与二维超声检查相比,采用ABUS在诊断乳腺微小结节良恶性中应用价值较高,能够明确结节位置,为临床诊断提供参考依据,值得推广应用。 Objective:To explore the diagnostic value of two-dimensional ultrasonography and automatic breast ultrasound diagnosis system(ABUS)in the differentiation of benign and malignant breast micronodules.Method:Eighty patients(111 nodules)admitted to our hospital from October 2018 to February 2020 who were confirmed to be breast micronodules were selected through pathological examination.All patients underwent two-dimensional ultrasound and ABUS examination.The detection of breast micronodules by different examination methods in each quadrant was compared,the application value of two-dimensional ultrasound and ABUS in the diagnosis of benign and malignant breast micronodules was analyzed,and the consistency of two-dimensional ultrasound and ABUS examination with pathological examination results was also analyzed.Result:The detection rate of breast micronodules of ABUS was higher than that of two-dimensional ultrasound,and the difference was statistically significant(P<0.05).The sensitivity,specificity and accuracy of ABUS in the differential diagnosis of benign and malignant breast micronodules were higher than those of two-dimensional ultrasound,and the differences were statistically significant(P<0.05).The Kappa test showed that the consistency between ABUS and pathological results was good(Kappa=0.891,P=0.000);the consistency between two-dimensional ultrasound and pathological results was not good(Kappa=0.396,P=0.000).Conclusion:Compared with two-dimensional ultrasound,the use of ABUS in the diagnosis of benign and malignant breast micronodules has a higher application value.It can clarify the position of the nodules and provide a reference for clinical diagnosis.It is worthy of promotion and application.
