超声内镜下细针穿刺液基细胞学对消化系统占位性病变的诊断价值

Diagnostic Value of Fluid Based Cytology of Fine Needle Puncture under Ultrasonic Endoscopy for Space-occupying Lesions of Digestive System

目的:探讨超声内镜下细针穿刺液基细胞学对消化系统占位性病变的诊断价值。方法:回顾性分析2020年12月-2021年5月笔者所在医院收治的188例消化系统占位性病变患者临床资料,所有患者均在笔者所在医院接受超声内镜下细针穿刺液基细胞学检测及传统细胞学检测。以术后病理学检查结果为金标准,比较两种检查方法对消化系统占位性病变的诊断效能,分析液基细胞学对消化系统恶性肿瘤病理分型的诊断准确率。结果:两种检查方法诊断消化系统占位性病变的漏诊率、误诊率、阴性预测值、阳性预测值、准确率、特异度、敏感度比较差异均无统计学意义(P>0.05)。液基细胞学在腺癌、恶性间皮瘤、淋巴瘤、鳞状细胞癌、神经内分泌癌、间质瘤、高级上皮内瘤变中的诊断准确率分别为83.61%(38/46)、50.00%(1/2)、50.00%(1/2)、83.33%(10/12)、50.00%(2/4)、28.57%(2/7)、50.00%(1/2)。结论:超声内镜引导下细针穿刺液基细胞学检测方法与传统细胞学对消化系统占位性病变具有同等的应用价值。

Objective:To investigate the diagnostic value of fluid based cytology of fine needle puncture under ultrasonic endoscopy for space-occupying lesions of digestive system.Method:The clinical data of 188 patients with space-occupying lesions of digestive system admitted in our hospital from December 2020 to May 2021 were analyzed retrospectively.All patients received liquid based cytology and traditional cytology of fine needle puncture under ultrasonic endoscopy examination in our hospital.The results of postoperative pathological examination were taken as the gold standard.The diagnostic efficiency of the two examination methods for space-occupying lesions of digestive system was compared,and the diagnostic accuracy of liquidbased cytology for pathological classification of digestive system malignant tumors was analyzed.Result:There were no significant differences in missed diagnosis rate,misdiagnosis rate,negative predictive value,positive predictive value,accuracy,specificity and sensitivity between the two examination methods in the diagnose of space-occupying lesions of digestive system(P>0.05).The diagnostic accuracy of liquid based cytology in adenocarcinoma,malignant mesothelioma,lymphoma,squamous cell carcinoma,neuroendocrine carcinoma,stromal tumor and advanced intraepithelial neoplasia were 83.61%(38/46),50.00%(1/2),50.00%(1/2),83.33%(10/12),50.00%(2/4),28.57%(2/7),50.00%(1/2).Conclusion:Fluid based cytology of fine needle puncture under ultrasonic endoscopy has the same application value as traditional cytology in space-occupying lesions of digestive system.
