摘要
目的:研究甲状腺良性结节患者应用超声引导下微波消融术治疗的效果,并评估其实施意义。方法:选取2018年6月-2019年6月本院确诊68例甲状腺良性结节患者,并将其随机分为对照组和研究组各34例,对照组实施常规手术治疗,研究组实施超声引导下微波消融治疗,对比两组术后3、6、12个月内的结节消退率、术中出血量、手术时间、住院时间、手术前后的甲状腺功能及术后并发症发生情况。结果:研究组术后3个月内的结节消退率100%,术后6个月内的结节消退率100%,均高于对照组的55.88%、67.65%(P<0.05),但两组术后12个月内的结节消退率对比差异无统计学意义(P>0.05);研究组术中出血量(1.27±0.36)ml,手术时间(32.15±5.79)min,住院时间(1.82±0.63)d,明显优于对照组的(6.32±1.21)ml、(60.21±14.47)min、(4.16±1.08)d(P<0.05)。术前,两组甲状腺功能差异无统计学意义(P>0.05);术后,研究组的TSH(2.12±0.51)mIU/L,FT_(4)(17.33±2.25)pmol/L,FT_(3)(4.89±0.92)pmol/L,均显著优于对照组的(4.67±0.48)mIU/L、(14.16±2.12)pmol/L、(3.13±0.71)pmol/L(P<0.05)。研究组并发症总发生率低于对照组,差异有统计学意义(P<0.05)。结论:甲状腺良性结节实施超声引导下微波消融术治疗,可提高结节消退率,改善患者甲状腺功能,促患者病情早日康复。
Objective:To study the effect of ultrasound-guided microwave ablation in the treatment of benign thyroid nodules and evaluate its significance.Method:A total of 68 patients with benign thyroid nodules diagnosed in our hospital from June 2018 to June 2019 were randomly divided into the control group and the study group,with 34 cases in each group.The control group was treated with routine surgery and the study group was treated with ultrasound-guided microwave ablation.The nodule regression rate,intraoperative bleeding,operation time,hospital stay and thyroid function and postoperative complications before and after operation.Result:The rates of nodule regression within 3 months and 6 months in the study group were 100%,which were higher than 55.88%and 67.65%in the control group(P<0.05),but there was no difference between the two groups within 12 months(P>0.05).The intraoperative blood loss(1.27±0.36)ml,operation time(32.15±5.79)min and hospital stay(1.82±0.63)d in the study group were significantly better than(6.32±1.21)ml,(60.21±14.47)min and(4.16±1.08)d in the control group(P<0.05).Before operation,there was no significant difference in thyroid function between the two groups(P>0.05).After operation,TSH(2.12±0.51)mIU/L,FT_(4)(17.33±2.25)pmol/L and FT_(3)(4.89±0.92)pmol/L in the study group were significantly better than(4.67±0.48)mIU/L,(14.16±2.12)pmol/L and(3.13±0.71)pmol/L in the control group(P<0.05).The total incidence of complication in the study group was lower than that in the control group(P<0.05).Conclusion:Ultrasound guided microwave ablation for benign thyroid nodules can improve the regression rate of nodules,improve the thyroid function and promote the early recovery of patients.
