崔云教授从肺论治前列腺疾病验案探析

Cases Analysis of Professor CUI Yun in Treating Prostatic Disorders from the Lung

[目的]总结探析崔云教授从肺论治前列腺疾病的学术思想及临床经验。[方法]跟随崔云教授临床侍诊,收集并列举分析崔云教授治疗慢性前列腺炎、前列腺增生、前列腺癌三则代表性验案,结合中医学经典理论,总结探析崔云教授从肺论治前列腺疾病的经验。[结果]基于中医藏象理论,崔云教授认为肺为五脏六腑之华盖,主行水,为水之上源,具有宣发肃降、通调水道的功能,肺与人体尿液代谢密切相关,因此临床上治疗前列腺疾病时常从肺卫不固、肺气郁闭、肺气亏虚等角度辨治,或辨症(即审症)论治、或辨体质论治、或辨病论治、或辨证论治。验案一中肺卫不固、肾虚湿热者治以补肺益肾、固精泄浊,验案二中肺气郁闭、湿热蕴结者治以开郁宣肺、清热利尿,验案三中肺气亏虚、痰瘀互结者治以温肺化痰、逐瘀消癥,治疗后临床疗效显著,为前列腺疾病的治疗开拓了新的思路与方法。[结论]崔云教授将五行藏象理论应用于男科临床,从肺论治前列腺疾病,结合并运用"审症-辨人(体质)-诊病-识证"的诊治模式,于常法中另辟蹊径,往往事半功倍,值得学习与借鉴。

[Objective]To summarize and analyze Professor CUI Yun’s academic thoughts and clinical experience of treating prostatic disorders from the lung.[Methods]Through following Professor CUI Yun’s clinical practice,this paper collected and analyzed three representative cases of Professor CUI Yun’s treatment of chronic prostatitis,prostatic hyperplasia and prostate cancer,combined with the classical theory of traditional Chinese medicine(TCM),summarized and analyzed Professor CUI Yun’s experience in treating prostatic disorders from the lung.[Results]Based on the theory of TCM viscera-state,Professor CUI Yun believes that the lungs are the canopy of the internal organs,the lungs dominate water,which is the source of water,and have the function of promulgating and subduing,they regulate the water channel and they are closely related to urine metabolism.Therefore,Professor CUI Yun often treats prostatic disorders from the perspectives of insufficient lung guarding,stagnation of lung Qi,and deficiency of lung Qi,based on symptom,or based on physique,or based on disease differentiation,or based on syndrome differentiation.In the first case,the patient with insufficient lung guarding,kidney deficiency and damp-heat was treated with the method of nourishing the lungs and kidneys,strengthening the essence and evacuating turbidity.In case two,the patient with stagnation of lung Qi and accumulation of damp-heat was treated with the method of opening depression and promoting the lung,clearing heat and diuresis.In case three,the patient with deficiency of lung Qi and mutual accumulation of phlegm and blood stasis was treated with the method of warming the lung to resolve phlegm,removing blood stasis and eliminating mass.After treatment,the clinical effect was significant,which opened up new ideas and methods for the treatment of prostatic disorders.[Conclusion]Professor CUI Yun applies the theory of the five elements and viscera-state of TCM in the andrology clinic to treat prostatic disorders from the lung,combining with and using the mode of"symptoms analysis-patients(physique)distinguish-disease diagnosis-syndrome identification",which is in the common method to find a new way,often gets twice the result with half the effort,it is worth learning and reference.
