浙派中医眩晕辨治思路探析

Exploration and Analysis on the Thoughts of Zhejiang School of TCM in the Differentiation and Treatment of Vertigo

[目的]探究浙派中医辨治眩晕的病因病机认识与治疗思路,总结其学术思想特色。[方法]通过查阅浙派中医中几位代表性医家的书籍、文献资料,摘录整理其关于眩晕病因病机、治则治法的论述,总结归纳浙派中医辨治眩晕的共同点及各家学术经验与特色。[结果]张景岳等医家认为眩晕多因正气亏虚,治疗应补虚为先,实脾补肾,慎用攻法。朱丹溪等医家主张因痰作眩,痰因火动,主要采用顺气治痰、兼降相火等治法。虞抟等则倡导瘀血致眩,重视活血化瘀法在眩晕中的应用,并且强调辨体论治。[结论]浙派中医辨治眩晕,病因病机立足虚、痰、瘀为核心,治疗上紧扣病因病机,攻补允当,为临床诊治眩晕提供了重要指导,其学术思想特色值得继承与发扬。

[Objective]To explore the thoughts of Zhejiang school of traditional Chinese medicine(TCM)in etiology pathogenesis and treatment and differentiation and treatment of vertigo,and summarize the academic characteristics.[Methods]By consulting books and literature data of several representative physicians in Zhejiang school of TCM,extracting and collating the discussions about etiology pathogenesis and treatment of vertigo,this paper summarized the common ground and each academic experience and characteristics of Zhejiang school of TCM in treating vertigo.[Results]ZHANG Jingyue and other physicians believed that vertigo was mostly caused by deficiency of the vital Qi,thus the treatment should be restoring firstly,invigorating the spleen and kidney,and using attack method carefully.ZHU Danxi and other physicians held that vertigo was due to phlegm and phlegm moved because of fire,mainly expounding the therapeutic methods of regulating Qi circulation to treat phlegm and descending ministerial fire.YU Tuan and others advocated blood stasis causing vertigo,and attached importance to apply the method of promoting blood circulation and removing blood stasis to treat vertigo,and laid emphasis on differentiation of individual constitution in differentiation and treatment.[Conclusion]In etiology and pathogenesis,Zhejiang school of TCM took deficiency,phlegm and blood stasis as the core of etiology and pathogenesis.In treatment,they conformed etiology and pathogenesis closely,benefited and eliminated properly.Zhejiang school of TCM provided important guidelines for clinical diagnosis and treatment of vertigo,and its academic characteristics are worth inheriting and carrying forward.
