四种miRNA在布鲁氏杆菌感染脊柱炎与脊柱结核患者诊治中的应用

Application of four miRNAs in the diagnosis and treatment of patients with brucellosis spondylitis and tuberculous spondylitis

目的探究微小RNA-150-5p(miR-150-5p)、miR-197-3p、miR-145-3p、miR-27b四种miRNA在布鲁氏杆菌感染致脊柱炎(BS)以及脊柱结核(TS)患者诊治中应用价值。方法以焦作市人民医院2016年7月-2019年7月收治的30例BS与32例TS作为研究对象,分别为BS组与TS组,同时选择同期于医院进行体检的健康者35名作为对照组。测定并比较外周血miR-150-5p、miR-197-3p、miR-145-3p、miR-27b四种miRNA表达水平及其在患者诊断治疗中临床意义。结果三组外周血miR-150-5p、miR-145-3p、miR-27b表达水平为TS组<BS组<对照组,而miR-197-3p表达水平为TS组>BS组>对照组(P<0.05);外周血miR-150-5p、miR-197-3p、miR-145-3p、miR-27b用于诊断脊柱结核曲线下面积(AUC)分别为0.938、0.880、0.729、0.528;BS组患者miR-150-5p、miR-197-3p、miR-145-3p、miR-27b水平与C-反应蛋白、红细胞沉降率、虎红平板凝集试验阳性、试管凝集试验滴度有关(P<0.05);BS组与TS组患者治疗后1年四种miRNA水平较治疗前改善(P<0.05),且治疗后1年两组患者上述四种miRNA水平与对照组比较差异无统计学意义。结论BS与TS患者体内miR-150-5p、miR-197-3p、miR-145-3p、miR-27b表达水平异常,可用于两者病情诊断以及治疗疗效评价。

OBJECTIVE To investigate the application value of the four miRNAs of microRNA(miR)-150-5p,miR-197-3p,miR-145-3p,and miR-27bin the diagnosis and treatment of patients with brucellosis spondylitis(BS)and tuberculous spondylitis(TS).METHODS Totally 30patients with BS(BS group)and 32patients with TS(TS group)treated in Jiaozuo People's Hospital from Jul.2016and Jul.2019were selected as the research subjects.Meanwhile,35healthy people who underwent physical examination in the hospital during the same period were selected as the control group.The expression levels of four miRNA of miR-150-5p,miR-197-3p,miR-145-3p,and miR-27bin peripheral blood were determined and compared,and their clinical significance in the diagnosis and treatment of patients was evaluated.RESULTS The expression levels of miR-150-5p,miR-145-3p and miR-27bin peripheral blood were the lowest in TS group,followed by BS group,and the highest in the control group.However,the expression level of miR-197-3p was the highest in TS group,followed by BS group,and the lowest in the control group(P<0.05).The AUC values of peripheral blood miR-150-5p,miR-197-3p,miR-145-3p,and miR-27bfor diagnosing TS were 0.938,0.880,0.729,and 0.528,respectively.The levels of miR-150-5p,miR-197-3p,miR-145-3p and miR-27bin the BS group were associated with C-reactive protein,erythrocyte sedimentation rate,buffered plate agglutination test positive,and tube agglutination test titer(P<0.05).Levels of the four kinds of miRNAs in both BS group and TS group were significantly improved one year after treatment compared with those before the treatment(P<0.05),and there was no significant difference in the level of the four miRNAs between the two groups of patients and the control group one year after treatment.CONCLUSION The expression levels of miR-150-5p,miR-197-3p,miR-145-3p,and miR-27blevels were abnormal in patients with BS and TS,which can be used for disease diagnosis and treatment efficacy evaluation of both.
