摘要
目的探讨早期梅毒及梅毒固定患者外周血单个核淋巴细胞中微小RNA-195(miR-195)、miR-223、miR-589的表达及临床价值。方法选择天津市滨海新区大港医院皮肤性病科2018年3月-2020年3月进行治疗的梅毒患者作为研究对象,根据其驱梅治疗后血清转变情况,从中选择血清转阴及血清固定患者各40例纳入研究,选择同期医院体检的健康人40名纳入对照组。采用快速血浆反应素实验(RPR)检测血清中梅毒抗体滴度;采用实时荧光定量聚合酶链式反应(RT-PCR)检测miR-195、miR-223、miR-589mRNA的表达情况。结果血清固定组miR-195、miR-223、miR-589水平高于正常转阴组及对照组(P<0.05);正常转阴组miR-195、miR-223、miR-589水平高于对照组(P<0.05);初始滴度<1∶8患者miR-195、miR-223、miR-589水平高于1∶8~1∶32患者及>1∶32患者(P<0.05);初始滴度1∶8~1∶32患者miR-195、miR-223、miR-589水平高于>1∶32患者,(P<0.05);临床分期为梅毒II期患者miR-195、miR-223、miR-589水平高于I期患者(P<0.001);受试者工作特征曲线分析结果显示,miR-195、miR-223、miR-589单指标用于梅毒血清固定诊断价值曲线下面积分别为0.725、0.753、0.749,联合诊断时曲线下面积为0.864。结论外周血miR-195、miR-223、miR-589在梅毒血清固定患者mRNA上表达显著上调,可能参与梅毒血清固定的免疫机制调节。
OBJECTIVE To explore the expression and clinical value of microRNA-195(miR-195),microRNA-223(miR-223)and microRNA-589(miR-589)in peripheral blood mononuclear lymphocytes of patients with early syphilis and syphilis fixation.METHODS The syphilis patients who were treated in the Department of Dermatology and Venereology,Dagang Hospital,Binhai New Area,Tianjin from Mar.2018to Mar.2020were selected as the research subjects.According to their seroconversion after syphilis treatment,40cases of serum-negative and serofixed patients were selected and included in the study,and 40healthy people who received physical examination in the hospital during the same period were selected as the control group.The rapid plasma reagin test(RPR)was used to detect the serum syphilis antibody titer;the real-time fluorescent quantitative polymerase chain reaction(RT-PCR)was used to detect the relative expression of miR-195,miR-223,and miR-589mRNA.RESULTS The levels of miR-195,miR-223and miR-589in the serum fixation group were significantly higher than those in the serum negative group and the control group(P<0.05);the levels of miR-195,miR-223and miR-589in the normal negative group were significantly higher than those in the control group(P<0.05);the levels of miR-195,miR-223,miR-589in patients with initial titer<1∶8were significantly higher than those in patients with titer of 1∶8~1∶32and>1∶32(P<0.05);The levels of miR-195,miR-223,and miR-589in patients with an initial titer of 1∶8~1∶32were significantly higher than those in patients with titer of>1∶32(P<0.05);the levels of miR-195,miR-223and miR-589of patients with clinically staging of syphilis phase II was significantly higher than those of patients with syphilis phase I(P<0.001);ROC analysis results showed that the area under the curve of the value of miR-195,miR-223,and miR-589for sero-fixed diagnostic value of syphilis was 0.725,0.753,and 0.749,respectively,and the area under the curve for combined diagnosis was 0.864.CONCLUSION Peripheral blood miR-195,miR-223,and miR-589were significantly up-regulated on the mRNA of syphilis serofixation patients,which may be involved in the immune mechanism regulation of syphilis serofixation.
