一种新型特制造口疝腹带的研制及应用评价

Development and application of a new type of specially abdominal belt of artificial fistula-induced hernia

目的:研制一种新型特制造口疝腹带,预防和保护术后造口旁疝的发生,增加患者造口佩戴的舒适感,并评价其临床应用价值。方法:在传统造口腹带的基础上对其进行改进,新型特制造口疝腹带针对男性与女性患者生理结构的不同设计不同款式,并选取医院收治的66例永久性肠造口患者,采用随机数表法将其分为观察组和对照组,每组33例,观察组使用新型特制造口疝腹带,对照组使用传统造口腹带,观察比较两组的造口腹带的佩戴效果,以及患者对腹带的满意度和依从性。结果:两组患者应用不同造口腹带后,观察组患者造口旁疝、造口周围皮肤并发症发生率分别为18.20%和6.06%,均低于对照组(45.45%和30.30%),差异有统计学意义(χ^(2)=5.66,χ^(2)=6.52;P<0.05);观察组患者对腹带的满意度优于对照组,差异有统计学意义(χ^(2)=4.69,P<0.05)。两组造口腹带佩戴依从性和不适感比较差异有统计学意义(χ^(2)2=6.52,χ^(2)=13.12;P<0.05)。结论:新型特制造口疝腹带具有固定稳固、个体化设计、量身定做、方法简单、配戴方便舒适以及不易移位等优势,可降低患者造口旁疝的发生,减少患者的医疗支出和经济负担,具有良好临床应用价值。

Objective:To develop a new type of specially abdominal belt of artificial fistula-induced hernia to prevent and protect the occurrence of postoperative artificial fistula-induced hernia and to increase the comfort of patients in wearing the artificial fistula.Methods:Based on the traditional abdominal belt of artificial fistula,the new specially abdominal belt of artificial fistula-induced hernia was improved and designed in different styles for male and female patients with different physiological structures.66 patients with perpetual artificial fistula on intestine who admitted to hospital were selected,and they were divided into observation group and control group as random number table,with 33 cases in each group.The observation group used the new special abdominal belt of artificial fistula-induced hernia,and control group used traditional abdominal belt of artificial fistula.The wearing effects of abdominal belt of artificial fistula,the satisfaction and compliance of patients between two groups were observed and compared.Results:After different abdominal belts of artificial fistula were applied in two groups,the incidences of artificial fistula-induced hernia and the complications of skin around artificial fistula of observation group were 18.20%and 6.06%,respectively,which were significantly lower than those(45.45%and 30.30%)of control group(x2=5.66,x2=6.52,P<0.05).And the satisfaction of the patients for abdominal belt of observation group was significantly better than that of control group(x2=4.69,P<0.05).The differences of the compliance of wearing abdominal belt of artificial fistula and the dis-comfortableness between two groups were significant(x2=6.52,x2=13.12,P<0.05).Conclusion:The new type of specially abdominal belt of artificial fistula-induced hernia has a series of advantages included stable fixation,individualized design,tailor-made,simple method,convenient and comfortable wearing and not easy to be displaced,which can decrease the occurrence of artificial fistula-induced hernia of patients and reduce the medical expenditure and economic burden of patients,and has favorable clinical application value.
