品管圈管理模式提高血液标本采集时间标记率降低血常规和CRP标本周转时间的研究 被引量:9

摘要 目的:运用品管圈(QCC)管理模式提高血液标本采集时间标记率,降低血常规和C反应蛋白(CRP)标本周转时间(TAT)。方法:成立检验科携手共进圈,选定“提高有效标本采集时间标记率,降低血常规和CRP标本TAT”为活动主题,绘制柏拉图和鱼骨图分析血液标本采集时间标记率低及血常规和CRP标本TAT平均较长的原因,设定改进目标值,并制定对策实施。结果:血液标本采集时间标记率低的主要原因为护士采血时不方便记录及对该记录意义认识不足,血常规和CRP标本TAT平均较长为标本送检时间集中及现有仪器通量小造成。执行相应改善措施后,有效标本采集时间标记率由15.42%提升至44.31%,血常规和CRP标本TAT中位数由156.58 min降至67.12 min,第90位百分数指标值由322.43 min降至115.83 min。结论:QCC模式能有效提高血液标本采集时间标记率,降低血常规和CRP标本TAT。 Objective:To improve the time recording rate of blood specimen sample collection and reduce the turnaround time(TAT)of routine blood test and CRP specimen through the quality control circle(QCC)management mode.Methods:The hand in hand circle of the clinical laboratory was set up."Improve the effective specimen collection time recording rate,reduce the TAT of routine blood and CRP specimen"was selected as the theme of the activity.Plato and fishbone diagram were drawn to analyze the reasons of low recording rate of blood sample collection time and the long average TAT of blood routine and CRP specimen.The improvement target value was set and countermeasures for implementation were formulated.Results:The main reasons for the low recording rate of blood sample collection time were the inconvenience of nurses to record blood collection time and the lack of understanding of the significance of the record.The longer average TAT of blood routine test and CRP specimen was resulted from the concentration of sample sending time and the low flux of existing instruments.After the implementation of the corresponding improvement measures,the effective sample collection time labeling rate increased from 15.42%to 44.31%,the median TAT of blood routine test and CRP decreased from 156.58 minutes to 67.12 minutes,and the 90th percentile decreased from 322.43 minutes to 115.83 minutes.Conclusion:The QCC mode can effectively improve the time recording rate of blood sample collection and reduce the TAT of blood routine test and CRP specimen.
