某肿瘤专科医院住院药房退药情况调查分析

Investigation and analysis of drug repercussion in inpatient pharmacy of a cancer hospital

目的分析住院药房退药情况以减少退药,促进合理用药。方法收集2019年7月~2020年6月退药数据,从退药原因、退药科室和退药种类3方面统计退药条目数和退药金额,分析退药现状。结果退药最多的退药原因依次为病情变化、医生修改医嘱、患者出院。退药金额最多的退药原因依次为医生修改医嘱、病情变化、患者拒用。退药排名前3的科室依次为放疗科、内科、肝脏胰胆外科,退药金额最大的前3个科室依次为内科、放疗科、鼻咽科。退药最多的3类药物为消化系统药物、营养药、抗微生物药物,金额占比最多的药品依次为抗肿瘤药物、血液及造血系统药物、消化系统药物。结论影响退药的因素很多,应从制度、信息系统、医患沟通等方面进行完善,加强退药管理,保障临床用药安全。

Objective The situation of drug repercussion in inpatient pharmacy was evaluated in order to reduce the affairs of drug repercussion and promote the rationality of drug use.Methods According to the data from July 2019 to June 2020,the status of drug repercussion were conducted on the items of drug repercussion and the drug refund in three aspects:the reasons,departments and types of drug repercussion.Results The changes of patient's condition was the most frequent reasons for drug repercussion,and followed by modifications of orders and the patient discharge.The modifications of orders was the first reason for drug refund followed by changes of patient's condition and patients’refusal.The top three departments in terms of the number of drug repercussion were radiotherapy department,internal medicine department and liver,pancreatic and biliary surgery department.The top three departments with the largest amount of medication refund were the department internal medicine,the department radiotherapy department and the nasopharynx department in turn.The top three type of medications with the most withdrawals were digestive system medication,nutritional medication and antimicrobial medication.The medications with the largest proportion of drug refund were anti-cancer medication,blood and hematopoietic medication and digestive medication in turn.Conclusion There are many factors influencing drug repercussion,which should be improved in terms of system,information system,doctor-patient communication and other aspects to strengthen the management of drug repercussion and ensure the safety of drug use.
