黄马酊联合三黄消炎方对糖尿病足溃疡创面修复的作用分析

Analysis of Effect of Huangma Tincture Combined with Sanhuang Xiaoyan Formula(三黄消炎方)on Diabetic Foot Ulcer

目的探讨黄马酊联合三黄消炎方对糖尿病足溃疡创面修复的作用。方法选择医院于2017年12月—2019年12月收治的糖尿病足溃疡患者98例,依据随机数字表法随机分为观察组49例与对照组49例。对照组患者给予黄马酊治疗,观察组在对照组基础上结合三黄消炎方治疗。两组疗程均为4周。比较两组治疗疗效,创面愈合情况,治疗前后溃疡面积、炎症因子、血清血管内皮生长因子(VEGF)、基质蛋白酶组织抑制剂(TIMP-1)、基质金属蛋白酶-2(MMP-2)水平变化,及不良反应情况。结果观察组总有效率(95.92%,47/49)高于对照组(81.63%,40/49)(P<0.05)。观察组创面愈合率高于对照组(P<0.05)。两组治疗后溃疡面积较治疗前减少(P<0.05);观察组治疗后溃疡面积小于对照组(P<0.05)。两组治疗后血清TNF-α、IL-1和ICAM-1水平较治疗前降低(P<0.05);观察组治疗后血清TNF-α、IL-1和ICAM-1水平低于对照组(P<0.05)。两组治疗后血清VEGF和TIMP-1水平较治疗前升高,血清MMP-2水平较治疗前降低(P<0.05);观察组治疗后血清VEGF和TIMP-1水平高于对照组,血清MMP-2水平低于对照组(P<0.05)。两组均未发生明显不良反应。结论黄马酊联合三黄消炎方对糖尿病足溃疡创面修复疗效良好,不仅可缩小创面面积,且可减轻炎症反应,下调MMP-2表达,及上调TIMP-1和VEGF表达。

e Objective To investigate the effect of Huangma Tincture combined with Sanhuang Xiaoyan Formula(三黄消炎方)on the repair of diabetic foot ulcer.s Methods A total of 98 patients with diabetic foot ulcer admitted in our hospital from December 2017 to December 2019 were randomly divided into observation group(49 cases)and control group(49 cases).The patients in the control group were treated with Huangma Tincture,and the observation group was treated with Sanhuang Xiaoyan Formula on the basis of the control group.The course of treatment in both groups was 4 weeks.The therapeutic effect,wound healing,ulcer area,inflammatory factors,serum levels of VEGF,TIMP-1 and MMP-2 and adverse reactions were compared before and after treatment.s Results The total effective rate of the observation group(95.92%,47/49)was higher than that of the control group(81.63%,40/49)(P<0.05).The wound healing rate of the observation group was higher than that of the control group(P<0.05).The ulcer area of the two groups after treatment was less than that before treatment(P<0.05).The ulcer area of the ob-servation group after treatment was less than that of the control group(P<0.05).The levels of TNF-α,IL-1 and ICAM-1 in the observation group were lower than those in the control group(P<0.05).After treatment,the levels of VEGF and TIMP-1 in serum of the two groups were higher than those before treatment,while the levels of MMP-2 in serum were lower than those before treatment(P<0.05).No significant adverse reactions occurred in both groups.n Conclusion Huangma Tincture combined with Sanhuang Xiaoyan Formula has a good curative effect on diabetic foot ulcer.It can not only reduce the wound area,but also reduce the inflammatory reaction,down regulate the level of MMP-2,and up regulate the levels of TIMP-1 and VEGF.
