不同浓度罗哌卡因在椎间孔镜术中腰麻的应用效果

Comparison of ropivacaine in different concentrations used as spinal anesthesia for percutaneous transforaminal endoscop⁃ic lumbar discectomy

[目的]比较不同浓度罗哌卡因在椎间孔镜下腰椎间盘摘除术腰麻中的应用效果。[方法]腰椎间盘突出症行椎间孔镜术的128例患者分为三组,腰麻中罗哌卡因浓度为0.1%的40例,0.15%的46例,0.2%的42例。比较三组患者麻醉效果。[结果]0.2%组患者麻醉平面达T10,而0.15%组在T11~T12,0.1%在L1~L3。麻醉效能的各指标均呈剂量依赖性(P<0.05)。[结论]0.15%罗哌卡因可能达到椎间孔镜椎间盘切除的良好麻醉平面,对运动神经阻滞较轻。

[Objective]To compare the clinical outcomes of ropivacaine in different concentrations used in spinal anesthesia for percu⁃taneous transforaminal endoscopic lumbar discectomy(PTELD).[Methods]A total of 128 patients who were undergoing PTELD for lum⁃bar disc herniation were divided into three groups.Of them,40 patients received spinal anesthesia with ropivacaine in the concentration of 0.1%,46 patients were with 0.15%,and the remaining 42 patients were with 0.2%.The anesthetic outcomes were compared among the 3 groups.[Results]The anesthesia level was at T10 in the 0.2%group,while between T11 and T12 in the 0.15%group,and between L1 and L3 in the 0.1%groups.All items of anesthesia efficacy were dose-dependent(P<0.05).[Conclusion]The 0.15%ropivacaine may reach a proper level of anesthesia for PTELD,with a benefit of lighter motor nerve block.
