烟台地区RhD阴性无偿献血者血型抗原分布调查及研究

Investigation on the blood group antigen distribution in RhD negative unpaid blood donors in Yantai area

目的了解烟台地区RhD阴性无偿献血者C、c、E、e抗原分布规律及表型,为献血者招募提供思路,同时为临床精准输血提供保障。方法收集2011年1月至2019年1月烟台地区经初筛RhD阴性无偿献血者1314例,共获取外周血样本2243例,通过抗球蛋白试验对样本进行RhD阴性确认,同时对经确认为RhD阴性的样本进行C、c、E、e抗原检测。统计分析烟台地区RhD阴性无偿献血者C、c、E、e抗原、表型及ABO血型分布频率。结果2243例经初筛为RhD阴性无偿献血者阴性确认率为99.06%(2222/2243);1314例确认为RhD阴性无偿献血者中献血1次914人,2次204人,3次72人,4次47人,5次26人,6次18人,7次16人,8次10人,9次5人,10次1人,11次1人;1314名RhD阴性献血者Rh血型C、c、E、e抗原阳性率分别为33.49%(440/1314)、95.36%(1253/1314)、7.69%(101/1314)和99.39%(1306/1314);Rh血型表型分布频率为dccee>dCcee>dccEe>dCCee>dCcEe>dccEE>dCCEe/dCcEE>dCCEE;RhD阴性无偿献血者ABO血型A∶B∶AB∶O为1.073∶1.590∶0.355∶1。结论获得烟台地区RhD阴性无偿献血者Rh血型C、c、E、e抗原及表型分布规律,建立RhD阴性血型库,为RhD阴性献血者招募及临床精准输血一定的提供依据。

Objective To understand the distribution and phenotype of C,c,E,e antigens in RhD negative unpaid blood donors in Yantai area,so as to provide ideas for the recruitment of blood donors and guarantee the clinical precision blood transfusion.Methods Total of 2243 peripheral blood samples from 1314 RhD negative unpaid blood donors in Yantai area were collected from January 2011 to January 2019.RhD negative samples were confirmed by antiglobulin test,and the C,c,E,e antigens were detected in RhD negative samples.C,c,E,e antigen,phenotype and ABO blood group distribution frequency of RhD negative unpaid blood donors in Yantai area were analyzed.Results The negative rate was 99.06%(2222/2243)in total of 2243 subjects who were screened as RhD negative unpaid blood donors.Among the 1314 unpaid blood donors confirmed RhD negative,there were 914 people donated for once,204 people donated for twice,72 people donated for three times,47 people donated for four times,26 people donated for five times,18 people donated for six times,16 people donated for seven times,10 people donated for eight times,5 people donated for nine times,1 people donated for eleven times.The positive rates of C,c,E,e antigens in 1314 RhD negative donors were 33.49%(440/1314),95.36%(1253/1314),7.69%(101/1314)and 99.39%(1306/1314)respectively.The distribution frequency of Rh blood group phenotype was ccdee>Ccdee>ccdEe>CCdee>CcdEe>ccdEE>CCdEe/CcdEE>CCdEE.The ABO blood group A∶B∶AB∶O of RhD negative volunteer blood donors was 1.073∶1.590∶0.355∶1.Conclusion The distribution law of Rh blood group C,c,E,e antigens and phenotype of RhD negative unpaid blood donors in Yantai area were obtained,and the RhD negative blood group bank was established.This may provide a basis for the recruitment of RhD negative blood donors and clinical precision blood transfusion.
