摘要
目的探讨血清尿酸、同型半胱氨酸、脂联素水平与阿尔茨海默病(AD)的关系。方法选择2019年3月-2020年10月本院收治的64例AD患者,另选择40例同年龄段健康体检人员作为对照组,收集所有研究对象临床资料,比较血清尿酸、同型半胱氨酸、脂联素水平,采用Logistic回归分析影响AD的因素,采用Pearson相关性方法分析血清学指标与MMSE评分的关系。结果 AD组糖尿病比例、同型半胱氨酸水平明显高于对照组(P <0.05),叶酸、尿酸、脂联素水平明显低于对照组(P <0.05);Logistic回归分析显示,糖尿病比例、尿酸、同型半胱氨酸、脂联素与AD显著相关(r值分别为9.903、6.797、10.316、18.015,P <0.05),叶酸与AD无显著相关性(P>0.05);不同病情程度患者尿酸、同型半胱氨酸、脂联素水平比较差异有统计学意义(P <0.05),即随着病情的加重,AD患者尿酸、脂联素水平呈降低趋势(P <0.05),同型半胱氨酸水平呈升高趋势(P <0.05);Pearson相关性分析显示,AD患者尿酸、脂联素水平与MMSE评分呈正相关(r值分别为0.750、0.759,P <0.05),同型半胱氨酸水平与MMSE评分呈负相关(r=-0.729,P <0.05)。结论血清尿酸、同型半胱氨酸、脂联素水平与AD发病风险和认知障碍程度关系密切,可为临床诊断、评估病情提供参考。
Objective To investigate the relationship between serum uric acid,homocysteine,adiponectin levels and Alzheimer’s disease( AD). Methods A total of 64 AD patients admitted to our hospital from March 2019 to October 2020 were selected.In addition,40 healthy medical examiners of the same age were selected as the control group,and clinical data of the patients were collected. Serum uric acid,homocysteine and adiponectin levels were compared,Logistic regression was used to analyze multiple factors influencing AD,and the relationship between serological indicators and MMSE score was analyzed by Pearson correlation method. Results The diabetes proportion and homocysteine in AD group were significantly higher than those in the control group( P < 0. 05),and the levels of folic acid,uric acid and adiponectin were significantly lower than those in the control group( P < 0. 05). Logistic regression analysis showed that the diabetes proportion,uric acid,homocysteine and adiponectin were significantly correlated with AD( r = 9. 903,r = 6. 797,r = 10. 316,r = 18. 015,P < 0. 05),while folic acid was not significantly correlated with AD( P > 0. 05). There was statistical significance on the differences in uric acid,homocysteine and adiponectin levels in patients with different degrees of disease( P < 0. 05). With the aggravation of the disease,the levels of uric acid and adiponectin decreased in AD patients( P < 0. 05),and the levels of homocysteine increased( P < 0. 05). Pearson correlation analysis showed that uric acid and adiponectin levels in AD patients were positively correlated with MMSE score( r =0. 750,r =0. 759,P <0. 05),and homocysteine levels were negatively correlated with MMSE score( r =-0. 729,P <0. 05). Conclusion Serum uric acid,homocysteine and adiponectin levels are closely related to the risk and cognitive function of AD,which may provide references for clinical diagnosis and evaluation of the disease.
