过敏源分析在瞬目增多儿童中的应用

Application of allergen analysis in children with increased blink

目的研究2013年-2018年浙江苍南县瞬目增多症状儿童的血清特异性IgE(specific Ig E,s Ig E)筛查情况、分析sIgE的分布及其结果为阳性儿童的临床特征,为临床病情的预防、诊断与治疗提供依据。方法本院近年来筛查出587例瞬目正常儿童和415例瞬目增多儿童,采用酶联免疫吸附试验检测血清sIgE,在不同年龄组间进行比较,并对sIgE结果阳性的瞬目增多儿童进行临床分型及特征汇总分析。结果 415份血清标本中s Ig E增高为69.88%(290/415),s Ig E增高儿童结合症状和体征诊断为过敏性结膜炎的65.54%(272/415)。导致瞬目增多的原因分型显示以常年性过敏性结膜炎为主,以眨眼、揉眼为主要症状。年龄分布以幼童组为主,所有患儿主要变应原分布以屋尘螨、粉尘螨、无爪螨,牛奶和鸡蛋白为主,吸入变应原s Ig E检出率高于食入变应原。多数患儿变应原阳性≥4种。结论过敏性结膜炎是引起儿童瞬目次数增加的重要原因,吸入性过敏原为过敏性结膜炎的主要变应原。

Objective To study the screening of serum-specific Ig E( s Ig E) in children with transiently increased symptoms in Cangnan County,Zhejiang Province from 2013 to 2018,to analyze the distribution of s Ig E and the clinical characteristics of positive children,and to provide basis for the prevention,diagnosis and treatment of clinical conditions. Methods The serum s Ig E of 587 normal children and 415 children screened in our hospital in recent years were compared with different age groups by enzyme linked immunosorbent assay. The clinical classification and characteristics of children with positive s Ig E results were analyzed. Results The increase in s Ig E in 415 serum samples was 69. 88%( 290/415),and 65. 54%( 272/415) of children with increased s Ig E combined with symptoms and signs were diagnosed as allergic conjunctivitis. The main causes of the increase were perennial allergic conjunctivitis,and the main clinical features were blinking and rubbing eyes. The age distribution was mainly in the young children group,and the main allergen distribution in all children was house dust mite,dust mite,clawless mite milk and egg white,and the detection rate of inhaled allergen s Ig E was higher than that of ingested allergen. Most children had no less than 4 positive allergens. Conclusion Allergic conjunctivitis is an important reason for the increase of blink number in children. Inhalation allergen is the main allergen of allergic conjunctivitis.
