中药治疗慢性疼痛并发抑郁症和焦虑症的效果研究

Effect of Traditional Chinese Medicine on Chronic Pain Complicated With Depression and Anxiety Disorder

目的探究中药辨证治疗慢性疼痛并发抑郁症和焦虑症的临床效果。方法本研究中的研究对象选择2018年2月—2019年2月本院收治的110例慢性疼痛并发抑郁症和焦虑症的患者,采用随机数字法将这110例患者分成中药组和西药组,两组均包括55例患者。根据患者的抑郁和焦虑的程度不同,对西药组患者给予不同的西药治疗,包括氟哌噻吨美利曲辛片(中度)和氟西汀及奥氮平(重度);根据患者的表证不同,对中药组患者给予不同的中药治疗,包括丹栀逍遥散(肝气郁结)、半夏厚朴汤(气滞痰瘀)和滋水清肝饮(阴虚火旺)。两组患者均治疗2个月,治疗结束后统计并观察比较两组患者的治疗效果、两组患者治疗前后的疼痛评分(VAS评分)和汉密尔顿抑郁量表(Hamilton depression scale,HAMD)以及汉密尔顿焦虑量表(Hamilton anxiety scale,HAMA)评分。结果本研究结果显示经过2个月的治疗,中药组患者的有效率显著高于西药组(P<0.05)。此外,治疗前,两组患者的VAS评分、HAMD评分和HAMA评分差异无统计学意义(P>0.05),但治疗后中药组患者的VAS评分、HAMA评分和HAMD评分均显著低于西药组患者(P<0.05)。结论相比较于西药治疗,采用中药辨证治疗慢性疼痛合并抑郁症和焦虑症的临床效果更好,患者的疼痛程度和抑郁焦虑程度更低。

Objective To explore the clinical effects of traditional Chinese medicine in the treatment of chronic pain complicated by depression and anxiety.Methods The subjects in this study selected 110 patients with chronic pain complicated by depression and anxiety who were admitted to our hospital from February 2018 to February 2019.These 110 patients were divided into Chinese medicine group and western medicine by random number method,both groups included 55 patients.According to the degree of depression of the patients,different western medicine treatments were given to patients in the western medicine group,including flupentixol and melitroxine tablets(moderate depression),fluoxetine and olanzapine(severe depression);according to the difference of patient’s manifestation,the patients in the traditional Chinese medicine group were given different traditional Chinese medicine treatments,including Danzhi Xiaoyaosan modified(Stagnation of Liver Qi),Banxia Houbu decoction(Qi stagnation and phlegm stasis)and Zishui Qinggan Yin modified(Yin Deficiency Huo Wang).Both groups of patients were treated for 2 months.After the treatment,the treatment effect of the two groups of patients was counted,observed and compared,including the pain score(VAS score),Hamilton depression scale(HAMD)and Hamilton anxiety scale(HAMA)of the two groups before and after treatment.Results The results of this study showed that after 2 months of treatment,the effective rate of the Chinese medicine group was significantly higher than that of the western medicine group(P<0.05).In addition,before treatment,the VAS score,HAMD score and HAMA score of the two groups of patients were not significantly different(P>0.05),but after treatment,the VAS score,HAMA score and HAMD score of the Chinese medicine group were significantly lower than those of the western medicine group(P<0.05).Conclusion Compared with western medicine treatment,the clinical effect of using traditional Chinese medicine to treat chronic pain combined with depression and anxiety is better,and the degree of pain and depression and anxiety of patients is lower.
