桂枝芍药知母汤在痛风性关节炎的应用及对血清IL-8、TGF-β的影响

The Application of Guizhi Shaoyao Zhimu Decoction in Gouty Arthritis and Its Influence on Serum IL-8 and TGF-β

目的观察桂枝芍药知母汤治疗痛风性关节炎的临床疗效及对血清中白细胞介素-8(Interleukin-8,IL-8)和转化生长因子-β(Transforming growth factor-β,TGF-β)的影响。方法选取2018年1月—2019年1月于江西省中医院风湿科住院部治疗的60例痛风性关节炎患者,每组30例。对照组予非布司他口服治疗,治疗组在对照组基础上予桂枝芍药知母汤内服治疗。比较2组治疗前后中医证候及血尿酸、血沉、C反应蛋白、IL-8及TGF-β水平变化,统计临床总体疗效。结果观察组的总有效率为90.00%(27/30),远远高于对照组的76.67%(23/30)(P<0.05);2组治疗后血尿酸、血沉、C反应蛋白及IL-8水平均较本组治疗前明显下降(P<0.05),且治疗组低于对照组(P<0.05),2组治疗后TGF-β水平均较本组治疗前明显上升(P<0.05),且治疗组高于对照组(P<0.05)。结论桂枝芍药知母汤治疗痛风性关节炎疗效显著,可明显改善症状。降低外周血中IL-8水平,增加TGF-β的表达水平。具有较好的抗炎、镇痛和降尿酸作用。

Objective To observe the clinical efficacy of Guizhi Shaoyao Zhimu decoction in the treatment of gouty arthritis and the effect on serum interleukin 8(IL-8)and TGF-β Methods 60 A total of 60 patients with gouty arthritis who were treated in Jiangxi Hospital of Traditional Chinese Medicine from January 2018 to January 2019 were selected,and they were divided into two groups,with 30 patients in each group.The control group was treated with febustata orally,and the treatment group was treated with Guizhi Shaoyao Zhimu decoction on the basis of the control group.Before and after the two groups of traditional Chinese medicine(TCM)symptoms and blood uric acid,esr,c-reactive protein,IL-8 and TGF-β levels were compared,the overall clinical efficacy was calculated.Results the total effective rate in the treatment group was 90.00%,and that in the control group was 76.67%.The difference in total effective rate between the two groups was statistically significant(P<0.05),the therapeutic effect of TCM symptoms in the treatment group was significantly better than that in the control group.The differences were statistically significant(P<0.05).After treatment,uric acid,esr,c-reactive protein and IL-8 levels in both groups were significantly lower than those before treatment in this group(P<0.05),and the treatment group was lower than the control group(P<0.05),TGF-βlevels in both groups were significantly higher after treatment than before treatment in this group(P<0.05),and the treatment group was higher than the control group(P<0.05).Conclusion Guizhi Shaoyao Zhimu decoction is effective in the treatment of gouty arthritis.The level of il-8 in peripheral blood was decreased and the expression level of TGF-β expression was increased.It has better anti-inflammatory,analgesic and uric acid function.
