基于方证思维血府逐瘀汤配合八髎穴提捏法治疗寒凝血瘀型痛经临床观察

Clinical Observation on Xuefu Zhuyu Decoction Combined with Baliao Acupoint Kneading Method in the Treatment of Dysmenorrhea with Cold-coagulation and Blood-stasis Stasis Based on Prescription Syndrome

目的观察基于方证思维血府逐瘀汤加减配合八髎穴提捏法治疗寒凝血瘀型痛经的临床疗效。方法选取苏州市中医医院2018年10月—2020年9月妇科门诊72例寒凝血瘀型原发性痛经患者,随机分为治疗组和对照组,各36例。治疗组以基于方证思维血府逐瘀汤加减配合八髎穴提捏法治疗,月经结束后口服中药至下1次月经开始,每日2次,经期以提捏八髎穴治疗,每日至少2次;对照组采用布洛芬缓释胶囊0.3 g/次,1日2次。观察2组治疗前后痛经临床症状评分变化以及治疗后2组临床总有效率。结果治疗后,治疗组患者痛经临床症状评分低于治疗前,且治疗组明显优于对照组,差异有统计学意义(P<0.05)。治疗组总有效率88.89%(32/36)高于对照组61.11%(22/36),差异有统计学意义(P<0.05)。结论本方法治疗寒凝血瘀型痛经疗效确切,方便简单,经期患者无需就医,居家操作;标本兼治,且避免了前列腺素合成酶抑制剂的不良反应,具有较高的临床应用及推广价值。

Objective To observe the clinical effect of Xuefu Zhuyu decoction combined with Baliao point kneading method in treating dysmenorrhea caused by cold coagulation and blood stasis;Methods Seventy-two patients with cold coagulation and blood stasis type primary dysmenorrhea in the department of Gynecology of Suzhou Hospital of Traditional Chinese Medicine from October 2018 to September 2020 were randomly divided into treatment group and control group,with 36 cases in each group.The treatment group was treated with the method of Xuefu Zhuyu decoction and Baliao acupoint.After menstruation,the traditional Chinese medicine was taken orally until the beginning of the next menstruation,twice a day.During menstruation,Baliao acupoint was treated at least twice a day.The control group received ibuprofen sustained-release capsules 0.3g/time,twice a day.The clinical symptom score of dysmenorrhea before and after treatment and the total clinical response rate after treatment were observed.Results After treatment,the clinical symptom score of dysmenorrhea in the treatment group was lower than before,and the treatment group was significantly better than the control group,the difference was statistically significant(P<0.05).The total effective rate in the treatment group was 88.89%(32/36)higher than that in the control group(61.11%,22/36),and the difference was statistically significant(P<0.05).Conclusion This method is effective in treating cold coagulation and blood stasis dysmenorrhea.It is convenient and simple.It can avoid the adverse reaction of prostaglandin synthase inhibitor,which has high clinical application and promotion value.
