腰椎间盘突出症患者住院费用结构及影响因素分析

Analysis on the Structure and Influencing Factors of Hospitalization Cost for Patients with Lumbar Intervertebral Disc Herniation

目的分析腰椎间盘突出症患者住院费用结构及其影响因素,为降低病种住院费用提供参考依据。方法通过某三级骨科专科医院2018年1月1日-2019年12月31日5576例腰椎间盘突出症住院患者病案首页,选取患者基本信息、诊断及费用信息,分析住院费用结构,利用独立样本t检验、χ^(2)检验、多元逐步回归分析对住院费用影响因素进行统计分析。结果住院费用主要组成部分是材料费(28.8%)、治疗费(22.6%)和检查费(19.4%),逐步回归分析结果显示腰椎间盘突出症患者住院费用的影响因素包括手术、住院天数,疑病例难、单病种、性别和年龄(P<0.01)。结论住院费用的主要组成部分是材料费,主要影响因素是手术和住院天数。严格把握手术适应症和手术指征以及缩短住院天数,可以有效降低腰椎间盘突出症患者住院费用。

Objectives To analyze the hospitalization costs structure and influencing factors in patients with lumbar disc herniation, and provide reference for reducing Hospitalization expenses. Methods Through a tertiary orthopaedic hospital on January 1 st, 2018 to December 31 st, 2019, 5576 cases of hospitalized patients with lumbar disc prolapse medical record front page, select patients’ basic information, cost information on diagnosis and analysis of the hospitalization cost structure, using independent sample t test, 2 test, multiple stepwise regression analysis on the factors affecting hospitalization cost for statistical analysis. Results The main components of hospitalization costs were the expenses for materials(28.8%), treatments(22.6%) and inspection(19.4%). The results of stepwise regression analysis showed that the factors affecting the cost were whether it was operated,hospitalization days, whether it was critical, whether it was single disease, gender and age(P<0.01).Conclusions The main component of hospitalization costs was the cost of materials, and the main influencing factors were whether surgery was performed and the length of stay. The cost of hospitalization for patients with lumbar disc herniation could be effectively reduced by reducing the cost of materials, strictly mastering the indications and indications of surgery and shortening the hospitalization days.
