摘要
目的探讨支气管哮喘患者血清分泌型卷曲蛋白5(SFRPS)水平与气道炎症、肺功能的相关性,为支气管哮喘病情判断、疗效评估提供依据。方法选取某院呼吸内科2018年2月1日-2020年1月1日收治的支气管哮喘患者127例,其中缓解期52例(缓解组),急性发作期75例(急性发作组),并选择健康体检者40例作为对照组。各组血清SFRP5、高敏C反应蛋白、白细胞介素-6、肿瘤坏死因子-α和免疫球蛋白E水平、呼出气一氧化氮含量和第1秒用力呼气容积占预计值的百分比、FVC/FEV1组间比较应用单因素方差分析,组间两两比较应用LSD-t检验。SFRP5与其他指标、病情程度的相关性应用Pearson相关分析。结果对照组、缓解组、急性发作组SFRP5、FEV1%pred、FVC/FEV1逐渐降低(P<0.01),hs-CRP、IL-6、IgE、TNF-α、FeNO逐渐升高(P<0.01),各指标组间两两比较差异均有统计学意义(P<0.01);急性发作组中,轻度、中度、重度患者SFRP5、FEV1%pred、FVC/FEV1逐渐降低(P<0.01),hs-CRP,IL-6,IgE、TNF-α、FeNO逐渐升高(P<0.01)各指标组间两两比较差异均有统计学意义(P<0.01);气管哮喘患者SFRP5与hs-CRP、IL-6、IgE、TNF-α、FeNO、病情程度均呈负相关(P<0.05),与FEV1%pred、FVC/FEV1均呈正相关(P<0.05);急性发作组患者治疗后SFRP5值均显著升高(P<0.01),重度组SFRP5高于轻度组、中度组(P<0.01)。结论SFRP5与气道炎症和病情呈负相关,与肺功能呈正相关,对支气管哮喘病情评估和疗效评价具有重要的参考价值。
Objectives To investigate the correlation between serum secreted curly protein 5 level and airway inflammation and pulmonary function in patients with bronchial asthma. Methods 127 patients with bronchial asthma admitted to the department of respiratory medicine of our hospital from February 1 st, 2018 to January 1 st, 2020 were selected, including 52 patients in remission stage(remission group) and 75 patients in acute attack stage(acute attack group), and 40 healthy patients were selected as the control group. One way ANOVA was used to compare serum SFRP5, high sensitivity C-reactive protein(hs-CRP), interleukin(IL)-6, tumor necrosis factor-α(TNF-α),immunoglobulin E(IgE) levels and fractional exhaled nitric oxide(FeNO) in each group were compared, as well as the percentage(FEV1%) of forced expiratory volume in the first second, and FVC/FEV1. LSD-t test was used to compare the two groups. Pearson correlation analysis was used to analyze the correlation between Sfrp5 and other indicators and disease severity. Results SFRP5, FEV1% pred, FVC/FEV1 decreased gradually in the control group, the remission group and the acute attack group(P<0.01), and hs-CRP, IL-6, IgE,TNF-αand FeNO increased gradually(P<0.01). The pairwise comparison between the groups of each index was statistically significant(P<0.01). In the acute attack group, SFRP5, FEV1% pred, FVC/FEV1 gradually decreased in patients with mild, moderate and severe disease(P<0.01), and hs-CRP, IL-6, IgE, TNF-α levels and FeNO gradually increased(P<0.01).The pairwise comparison between the groups of each index was statistically significant(P<0.01). In patients with tracheal asthma, SFRP5 was negatively correlated with hs-CRP, IL-6,IgE, TNF-α, FeNO and the severity of the disease(P<0.05), and positively correlated with FEV1% pred and FVC/FEV1(P < 0.05). The SFRP5 value of the acute attack group was significantly increased after treatment(P<0.01), and that of the severe group was higher than that of the mild and moderate groups(P<0.01).Conclusions SFRP5 was negatively correlated with airway inflammation and disease, and positively correlated with pulmonary function, so it had important reference value for condition evaluation and efficacy evaluation of bronchial asthma.
