消疕汤对银屑病患者外周血免疫细胞平衡及NF-κB、Lp-PLA2的影响

Effect of Xiaobi Decoction on Peripheral Blood Immune Cell Balance and NF-κB,Lp-PLA2 in Patients with Psoriasis

目的:探究消疕汤结合阿维A对银屑病患者外周血免疫细胞平衡及核转录因子-κB(NF-κB)、血清脂蛋白相关磷脂酶A2(Lp-PLA2)的影响。方法:选取符合纳入标准的银屑病患者106例按照随机数字表分为研究组53例(消疕汤+阿维A治疗)与对照组53例(阿维A治疗)。比较两组治疗疗效、外周血免疫细胞平衡、炎症因子、银屑病面积与严重性指数(PASI)及NF-κB、Lp-PLA2水平的差异。结果:研究组治疗2、3、4周后的总有效率均高于对照组,组间差异具有统计学意义(Wald χ^(2)_(组别)=8.000,Wald χ^(2)_(时点)=64.242,P_(组别)=0.005,P_(时点)<0.001)。治疗前两组患者IFN-γ、TNF-α、IL-17、IL-22、IL-4、CD_(3)^(+)、CD_(4)^(+)、CD_(4)^(+)/CD_(8)^(+)、NF-κB、Lp-PLA2水平、PASI评分比较,均无统计学意义(P>0.05);治疗后两组患者IFN-γ、TNF-α、IL-17、IL-22、IL-4、NF-κB、Lp-PLA2水平、PASI评分均低于治疗前,CD_(3)^(+)、CD_(4)^(+)、CD_(4)^(+)/CD_(8)^(+)高于治疗前(P<0.05),且研究组改善情况优于对照组(P<0.05)。结论:消疕汤联合阿维A对银屑病患者疗效显著,可有效促进患者机体免疫平衡,降低患者机体炎症反应,提高机体免疫力。

Objective:To investigate the effects of Xiaobi Decoction combined with Acitretin Capsule on peripheral blood immune cell balance and nuclear transcription factor-κB(NF-κB)and serum lipoprotein-associated phospholipase A2(Lp-PLA2)in patients with psoriasis.Methods:106 patients with psoriasis were randomly divided into the study group(n=53)and the control group(n=53).The control group was treated with Acitretin Capsule;on which basis,the treatment group was also treated with Xiaobi Decoction.Therapeutic efficacy,peripheral blood immune cell balance,as well as the levels of NF-κB and Lp-PLA2 were compared between the two groups after the treatment.Results:The total effective rate of the study group was higher than that of the control group after 2 w,3 w and 4 w of the treatment(Wald χ^(2)_(group)=8.000,Wald χ^(2)_(time)=64.242,P_(group)=0.005,P_(time)<0.001).There were no statistical differences in the levels of IFN-γ,TNF-α,IL-17,IL-22,IL-4,CD_(3)^(+),CD_(4)^(+),CD_(4)^(+)/CD_(8)^(+),NF-κB,Lp-PLA2 and PASI score between the two groups before the treatment(P>0.05).After the treatment,the levels of IFN-γ,TNF-α,IL-17,IL-22,IL-4,NF-κB and Lp-PLA2,as well as the score of PASI were lower,and the levels of CD_(3)^(+),CD_(4)^(+) and CD_(4)^(+)/CD_(8)^(+) were higher than those before the treatment in the two group(P<0.05),of which the improvements were more significant in the study group(P<0.05).Conclusion:The curative effect of Xiaobi Decoction combined with Acitretin Capsule is significant in the treatment of psoriasis.It can effectively promote immune balance,reduce inflammatory reactions and improve immunity of the patients.
