超声心动图声学造影在肺动静脉瘘诊断中的临床应用价值分析

Analysis on the Clinical Application Value of Contrast-Enhanced Echocardiography in the Diagnosis of Pulmonary Arteriovenous Fistula

目的分析超声心动图声学造影在肺动静脉瘘诊断中的临床应用价值。方法选取2018年6月至2020年7月我院收治的73例疑似肺动静脉瘘患者,均行超声心动图声学造影检测,并以数字减影血管造影技术(DSA)检查结果为标准,分析超声心动图声学造影在肺动静脉瘘中的诊断价值。结果经DSA检查结果显示,73例疑似肺动静脉瘘患者中确诊62例。超声心动图声学造影诊断肺动静脉瘘的结果与DSA诊断结果的一致性较好(κ=0.846,P=0.000)。超声心动图声学造影诊断肺动静脉瘘的灵敏度为96.77%,准确度为95.89%,特异度为90.91%;超声心动图声学造影的表现中,推注声学对比剂后,肺动静脉瘘患者右心先充盈、先排空,而左心后充盈、后排空。结论超声心动图声学造影在肺动静脉瘘诊断中具有较高的应用价值,临床可结合超声心动图声学造影影像学特征筛选出肺动静脉瘘患者,并制定合理的治疗措施以有效改善患者预后。

Objective To analyze the clinical application value of contrast-enhanced echocardiography in the diagnosis of pulmonary arteriovenous fistula.Methods 73 patients with suspected pulmonary arteriovenous fistula admitted to our hospital from June 2018 to July 2020 were selected and given contrast-enhanced echocardiography.Taking the digital subtraction angiography(DSA)results as the standard,the diagnostic value of contrast-enhanced echocardiography in pulmonary arteriovenous fistula was analyzed.Results DSA results showed that among 73 cases of patients with suspected pulmonary arteriovenous fistula,62 cases were confirmed.The results of contrast-enhanced echocardiography in the diagnosis of pulmonary arteriovenous fistula were consistent with DSA results(κ=0.846,P=0.000).The sensitivity,accuracy and specificity of contrast-enhanced echocardiography in the diagnosis of pulmonary arteriovenous fistula were 96.77%,95.89%and 90.91%,respectively.In contrast-enhanced echocardiography,after injection of contrast agent,the right heart of patients with pulmonary arteriovenous fistula was filled and emptied first,while the left heart was filled and emptied later.Conclusions Contrast-enhanced echocardiography has higher application value in the diagnosis of pulmonary arteriovenous fistula.The clinical features of contrast-enhanced echocardiography can be combined to screen out patients with pulmonary arteriovenous fistula,and formulate reasonable treatment measures to effectively improve the prognosis of patients.
