全身麻醉联合髂筋膜间隙阻滞在髋关节置换手术患者中的应用

Application of General Anesthesia Combined with Iliac Fascia Space Block in Patients Undergoing Hip Replacement

目的探讨全身麻醉联合髂筋膜间隙阻滞在髋关节置换手术患者中的应用效果。方法选取2018年3月至2020年3月于我院行髋关节置换手术的90例患者,随机分为对照组(n=45)和观察组(n=45)。对照组行全身麻醉,观察组在对照组基础上行髂筋膜间隙阻滞,比较两组患者术前及术后24 h的疼痛介质水平及炎性因子水平。结果术后24 h,两组的NPY、PGE_(2)、5-HT、β-EP、SP水平均升高,观察组的NPY、PGE_(2)、5-HT、β-EP、SP水平低于对照组(P<0.05)。术后24 h,两组的CRP、IL-1β、TNF-α水平均升高,观察组的CRP、IL-1β、TNF-α水平低于对照组(P<0.05)。结论全身麻醉联合髂筋膜间隙阻滞可有效提高髋关节置换手术患者的镇痛效果,降低其炎性应激反应。

Objective To analyze the application effect of general anesthesia combined with iliaca fascia space block in patients undergoing hip replacement.Methods 90 patients undergoing hip replacement in our hospital from March 2018 to March 2020 were selected and randomly divided into control group(n=45)and observation group(n=45).The control group was given general anesthesia,and the observation group was given iliac fascia space block on the basis of the control group.The levels of pain mediators and inflammatory factors before surgery and 24 hours after surgery were compared between the two groups.Results 24 hours after surgery,the NPY,PGE_(2),5-HT,β-EP and SP levels of the two groups increased,but the NPY,PGE_(2),5-HT,β-EP and SP levels of the observation group were lower than those of the control group(P<0.05).24 hours after surgery,the CRP,IL-1βand TNF-αlevels of the two groups increased,but the CRP,IL-1βand TNF-αlevels of the observation group were lower than those of the control group(P<0.05).Conclusions General anesthesia combined with iliac fascial space block can effectively improve the analgesic effect of patients undergoing hip replacement and reduce the inflammatory stress response.
