肝功能与血常规指标联合检测在乙型肝炎临床诊断中的价值

Value of Combined Detection of Liver Function and Blood Routine Indicators in the Clinical Diagnosis of Hepatitis B

目的探讨肝功能与血常规指标联合检测在乙型肝炎临床诊断中的价值。方法选取2019年12月至2021年1月我院收治的56例乙型肝炎患者作为观察组,另选取同期56例健康体检者作为对照组。检测两组的肝功能指标与血常规指标,并绘制ROC图像分析各指标对乙型肝炎的诊断价值。结果观察组白细胞、血小板计数与中性粒细胞水平明显低于对照组,淋巴细胞水平明显高于对照组(P<0.05)。观察组天冬氨酸转氨酶、丙氨酸转氨酶、总胆红素水平明显高于对照组(P<0.05)。ROC曲线图分析结果显示,肝功能与血常规指标联合检测诊断乙型肝炎的曲线下面积(AUC)、灵敏度、特异度分别为0.879、89.95%、92.36%,均高于任意单一指标(P<0.05)。结论乙型肝炎患者发病期间血常规指标变化明显,肝功能存在不同程度损伤,二者联合检测有利于提高乙型肝炎的诊断效能。

Objective To explore the value of combined detection of liver function and blood routine indicators in the clinical diagnosis of hepatitis B.Methods 56 patients with hepatitis B admitted to our hospital from December 2019 to January 2021 were selected as observation group,and 56 healthy people at the same period were selected as control group.The liver function indicators and blood routine indicators of the two groups were detected,and the ROC image was drawn to analyze the diagnostic value of each index for hepatitis B.Results The leukocyte,platelet count and neutrophil levels of the observation group were significantly lower than those of the control group,and the lymphocyte level was significantly higher than that of the control group(P<0.05).The levels of aspartate aminotransferase,alanine aminotransferase and total bilirubin in the observation group were significantly higher than those in the control group(P<0.05).ROC curve analysis showed that the area under the curve(AUC),sensitivity and specificity of combined detection of liver function and blood routine indicators in the diagnosis of hepatitis B were 0.879,89.95%and 92.36%respectively,higher than any single indicator(P<0.05).Conclusions During the onset of hepatitis B,the changes of blood routine indicators are obvious,and the liver function is damaged in different degrees.The combined detection is helpful to improve the diagnostic efficiency of hepatitis B.
