塞来昔布胶囊联合硫酸氨基葡萄糖胶囊对膝骨性关节炎临床研究

Clinical study of celecoxib capsule combined with glucosamine sulfate capsule on patients with knee osteoarthritis

目的:探讨塞来昔布胶囊联合硫酸氨基葡萄糖胶囊对膝骨性关节炎患者疗效及对骨代谢和炎性因子影响。方法:选择抚州市第一人民医院于2018年5月—2021年2月膝骨性关节炎患者60例,依据随机表法分为观察组30例与对照组30例。对照组口服硫酸氨基葡萄糖胶囊1粒/次,3次/d;观察组在硫酸氨基葡萄糖胶囊基础上口服塞来昔布胶囊200 mg/次,1次/d。两组治疗疗程12周。比较两组治疗12周总有效率、两组治疗前后疼痛视觉模拟评分法(VAS)和膝关节功能量表(Lysholm)评分、骨代谢[β胶原降解产物(β-CTX)、特异性碱性磷酸酶(BALP)和骨钙素(BGP)]和炎性因子[白介素-1(IL-1)、白介素-6(IL-6)和肿瘤坏死因子-α(TNF-α)]变化以及不良反应。结果:观察组治疗12周膝骨性关节炎患者总有效率高于对照组(P<0.05)。两组治疗前患者VAS评分、Lysholm评分、β-CTX、BALP、BGP、IL-1、IL-6和TNF-α水平比较差异无统计学意义(P>0.05);两组治疗12周患者VAS评分、β-CTX、IL-1、IL-6和TNF-α水平低于治疗前,而Lysholm评分、BALP和BGP水平高于治疗前(P<0.05);观察组治疗12周患者VAS评分、β-CTX、IL-1、IL-6和TNF-α水平低于对照组,而Lysholm评分、BALP和BGP水平高于对照组(P<0.05)。治疗期间两组均未出现明显不良反应。结论:塞来昔布胶囊联合硫酸氨基葡萄糖胶囊对膝骨性关节炎患者疗效良好,且可改善患者骨代谢,减轻炎性反应。

Objective:To investigate the effect of celecoxib capsule combined with glucosamine sulfate capsule on patients with knee osteoarthritis and its influence on bone metabolism and inflammatory factors.Methods:A total of 60 patients with knee osteoarthritis in our hospital from May 2018 to February 2021 were selected and divided into the observation group and the control group(n=30)according to the random table method.The control group received glucosamine sulfate capsules one capsule/time,three times a day.The observation group was orally given celecoxib capsules 200 mg/time,once a day based on glucosamine sulfate capsules.Both groups were treated for 12 weeks.After 12 weeks of treatment,the total effective rate was compared between the two groups.Pain visual analog scale(VAS)scores,knee function scale(HSS)scores,bone metabolism[β-collagen degradation products(β-CTX),specific alkalinity phosphatase(BALP),and osteocalcin(BGP)],and inflammatory factors[interleukin-1(IL-1),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)],and adverse reactions before and after treatment were compared between both groups.Results:The total effective rate of the observation group was higher than that of the control group(P<0.05).There was no significant difference in the levels of VAS score,Lysholm score,β-CTX,BALP,BGP,IL-1,IL-6,and TNF-αin patients with knee osteoarthritis before treatment(P>0.05).After 12 weeks of treatment,the VAS score,β-CTX,IL-1,IL-6,and TNF-αlevels in both groups were lower than those before treatment(P<0.05),while the Lysholm score,BALP,and BGP levels were higher than those before treatment(P<0.05).The VAS score,β-CTX,IL-1,IL-6,and TNF-αlevels in the observation group were lower than those in the control group,while the Lysholm score,BALP,and BGP levels were higher than those in the control group(P<0.05).There were no obvious adverse reactions in both groups during the treatment.Conclusion:Celecoxib capsules combined with glucosamine sulfate capsules have a good effect on patients with knee osteoarthritis and can improve bone metabolism and reduce inflammatory reactions.
