摘要
目的:探讨医务人员感知医患关系与工作认同之间的关系及工作压力、离职意愿、工作满意在其间的作用。方法:利用全国第六次卫生服务调查上海市部分数据进行验证性因子分析、多组独立样本秩和检验、Spearman秩相关分析,构建结构方程模型并进行多群组分析探索中介作用和调节作用。结果:公立医院医务人员感知医患关系显著负向影响离职意愿和工作压力,工作压力不仅对离职意愿有显著影响,且在感知医患关系和离职意愿之间起到中介作用,工作认同和工作满意在模型中均起到调节作用。结论:医务人员感知医患关系可以作为工作压力评价的维度之一;工作认同和工作满意分别通过中介过程的后半路径和前半路径起到调节作用;考虑不同性别及医务人员的工作压力特征为公立医院人才保留机制建设提出新方向。
Objective To explore the relationship between the perceived doctor-patient relationship and career identity of medical staffs,and the role of occupational stress,turnover intention and job satisfaction in it.Methods Partial data from the sixth national health service survey in Shanghai was studied with factor analysis,multi-group independent sample rank sum test,spearman rank correlation analysis and a structural equation model.Results The medical professionals’perceived doctor-patient relationship can significantly negatively affect their turnover intention and occupational stress.The occupational stress not only has a significant impact on the turnover intention,but also played an intermediary role between the perceived doctor-patient relationship and turnover intention.And career identity and job satisfaction play a moderating role in the model.Conclusion The medical professionals’perceived doctor-patient relationship may be one of the dimensions of occupational stress evaluation.And the moderating effect can be realized through the indirect path by career identity and job satisfaction.Considering gender and occupational stress provides a new direction for the construction of talent retaining mechanism in public hospitals.
