某院采用国家带量采购口服降糖药物的效果与相关问题分析

Analysis on the Effectiveness and Related Problems of the National Volume-Based Procurement of Oral Hypoglycemic Drugs in a Hospital

目的分析平顶山市第一人民医院采用国家带量采购的口服降糖药物的价格、销售情况和相关用药咨询问题,评价实施国家药品集中采购政策的效果,为政策的完善提供相应的建议。方法收集2019年1—6月平顶山市第一人民医院统一招标的口服降糖药(未采购"4+7"药品)规格数量、厂家、价格、销售量、供货单位,收集2021年1—6月平顶山市第一人民医院统一招标的口服降糖药(含采购"4+7"药品)规格数量、厂家、价格、销售量、供货单位。收集平顶山市第一人民医院2019年1—6月和2021年1—6月的用药咨询记录问题种类数量。结果医院采用国家带量采购口服降糖药后,口服降糖药价格降幅明显,并且同剂量单片降价幅度均在62%以上,其中格列美脲片降幅为93.75%,居首位。口服降糖药销量数量呈增长趋势,总销售金额下降77.24%,供货单位增加至3家,带量采购药品的可替代性用药咨询问题数量呈上升趋势。结论国家带量采购药品可有效减轻患者的经济负担,改善临床合理选药,但仍存在部分问题,需要进一步探讨。

Objective To analyze the price,sales status and related medication consultation of the national volume-based procurement of oral hypoglycemic drugs in the First People’s Hospital of Pingdingshan,and evaluate the effectiveness of implementing the national volume-based procurement policy and provide some feasible suggestions in order to perfect the national volume-based procurement of policy.Methods Collect the specifications,quantity,manufacturer,price,sales volume and supplier of oral hypoglycemic drugs uniformly tendered by the First People’s Hospital of Pingdingshan from January to June 2019(without"4+7"drugs),and collect the specifications,quantity,manufacturer,price,sales volume and supplier of oral hypoglycemic drugs uniformly tendered by the First People’s Hospital of Pingdingshan from January to June 2021(including“4+7”drugs).Collect the number of problem types of the medication consultation records of the First People’s Hospital of Pingdingshan from January to June 2019 and from January to June 2021.Results After the hospital purchased oral hypoglycemic drugs by the national volume-based procurement,the price of oral hypoglycemic drugs decreased significantly,and the price reduction of single tablet at the same dose was more than 62%,of which glimepiride tablet decreased by 93.75%,ranking the first.The sales volume of oral hypoglycemic drugs showed an increasing trend,and the total sales amount decreased by 77.24%,while the number of suppliers increased to 3,and the number of alternative medication consultation questions with the national volume-based procurement showed an upward trend.Conclusion The national volume-based procurement of oral hypoglycemic drugs can effectively reduce the financial burden of patients and improve clinical rational drug selection,however there are some problems that need to be further discussed.
