乳腺癌患者改良根治术后发生疼痛综合征的影响因素

Factors of Pain Syndrome After Modified Radical Mastectomy in Patients with Breast Cancer

目的探讨乳腺癌患者改良根治术后发生疼痛综合征的影响因素。方法收集2019年1月至2021年7月商丘市第一人民医院收治的50例接受改良根治术后发生疼痛综合征的乳腺癌患者的临床资料,设为发生组;另收集同期商丘市第一人民医院收治的50例接受改良根治术后未发生疼痛综合征的乳腺癌患者的临床资料,设为未发生组。查阅并收集患者的病历资料,经单因素与多因素分析明确乳腺癌患者改良根治术后发生疼痛综合征的影响因素。结果发生组术中腋窝淋巴结清扫、肿瘤所处部位、术后放疗、术前焦虑及抑郁程度与未发生组比较,差异有统计学意义(P<0.05);经logistic回归分析结果显示,肿瘤处于上象限、术中行腋窝淋巴结清扫、术后放疗、术前焦虑及抑郁严重是乳腺癌患者改良根治术后发生疼痛综合征的影响因素(OR>1,P<0.05)。结论肿瘤处于上象限、术中行腋窝淋巴结清扫、术后放疗、术前焦虑及抑郁严重是乳腺癌患者改良根治术后发生疼痛综合征的影响因素。

Objective To investigate the influencing factors of pain syndrome after modified radical mastectomy in patients with breast cancer.Methods The clinical data of 50 patients with breast cancer who had pain syndrome after modified radical mastectomy in Shangqiu First People’s Hospital from January 2019 to July 2021 were collected and set as the occurrence group.The clinical data of 50 patients with breast cancer who did not have pain syndrome after modified radical mastectomy in Shangqiu First People’s Hospital during the same period were collected and set as the non-occurrence group.The medical records of patients were reviewed and collected.Single factor and multiple factors analysis were used to find out the influencing factors of pain syndrome after modified radical mastectomy in breast cancer patients.Results There were statistical differences in intraoperative axillary lymph node dissection,tumor location,postoperative radiotherapy,preoperative anxiety and depression between occurrence group and non-occurrence group(P<0.05).Logistic regression model analysis results showed that the upper quadrant,axillary lymph node dissection,postoperative radiotherapy,preoperative anxiety and depression were the influencing factors of pain syndrome after modified radical mastectomy in breast cancer patients(OR>1,P<0.05).Conclusion The upper quadrant,axillary lymph node dissection,postoperative radiotherapy,preoperative anxiety and depression are the influencing factors of pain syndrome after modified radical mastectomy in breast cancer patients.
