老年股骨颈骨折患者内固定术后正念水平的影响因素

Influencing Factors of Mindfulness Level in Elderly Patients with Femoral Neck Fracture After Internal Fixation

目的探讨老年股骨颈骨折(FNF)患者内固定术后正念水平的影响因素。方法选取2018年3月至2021年4月息县人民医院收治的87例接受内固定术治疗的老年FNF患者为研究对象,于术后1周时,采用正念注意觉知量表评估所有患者正念水平;记录患者入院时基线资料,分析老年FNF患者内固定术后正念水平的影响因素。结果经评估,87例老年FNF患者固定术后正念水平较好39例,占比44.83%(39/87),正念水平中等、较差共48例,占比55.17%(48/87);正念水平中等、较差的患者慢性病数量多于正念水平较好的患者,家庭关怀度指数问卷(APGAR)评分、社会支持评定量表(SSRS)评分均低于正念水平较好的患者,差异有统计学意义(P<0.05);经logistic回归分析发现,慢性病数量多为老年FNF患者内固定术后正念水平低下的影响因素(OR>1,P<0.05),APGAR评分及SSRS评分高均为老年FNF患者内固定术后正念水平的保护因素(OR<1,P<0.05)。结论老年FNF患者内固定术后正念水平一般,而慢性病数量多、社会支持度与家庭关怀度低均可能为老年FNF患者内固定术后正念水平的影响因素。

Objective To explore the influencing factors of mindfulness level after internal fixation in elderly patients with femoral neck fracture(FNF).Methods Eighty-seven elderly FNF patients treated with internal fixation in Xi County People’s Hospital from March 2018 to April 2021 were selected as the research subjects.One week after operation,the mindfulness level of all patients was evaluated by mindfulness attention awareness scale.The baseline data of patients at admission were recorded,and the influencing factors of mindfulness level in elderly patients with FNF after internal fixation were analyzed.Results After assessment,39 of 87 elderly patients with FNF after internal fixation had good mindfulness level,accounting for 44.83%(39/87),and 48 had medium and poor mindfulness level,accounting for 55.17%(48/87).The number of chronic diseases in patients with moderate and poor mindfulness level was more than that in patients with better mindfulness level,and the scores of family care index questionnaire(APGAR)and social support rating scale(SSRS)were lower than those in patients with better mindfulness level(P<0.05).Logistic regression analysis showed that large number of chronic diseases was the influencing factor of low mindfulness level after internal fixation in elderly patients with FNF(OR>1,P<0.05),and high APGAR score and SSRS score were protective factors for mindfulness level after internal fixation in elderly patients with FNF(OR<1,P<0.05).Conclusion The level of mindfulness in elderly patients with FNF after internal fixation is general.The large number of chronic diseases,low social support and family care may be the influencing factors of mindfulness level after internal fixation in elderly patients with FNF.
