弹拨牵拉胸大肌为主推拿手法联合针刺治疗粘连期肩周炎患者的效果

Effect of Plucking and Pulling the Pectoralis Major Muscle as the Main Massage Combined with Acupuncture in the Treatment of Patients with Periarthritis Humeroscapularis at the Adhesion Stage

目的分析弹拨牵拉胸大肌为主推拿手法联合针刺治疗粘连期肩周炎患者的效果。方法选取2019年3月至2020年10月平顶山市第二人民医院收治的84例粘连期肩周炎患者,按照随机数字表法分为观察组(42例)和对照组(42例)。其中对照组接受针刺治疗,观察组接受弹拨牵拉胸大肌为主推拿手法联合针刺。对比两组疗效、视觉模拟评分法(VAS)评分、Constant-Murley肩关节评分及血清炎症因子[白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、单核细胞趋化蛋白-1(MCP-1)及白细胞介素-10(IL-10)]水平。结果观察组总有效率[95.24%(40/42)]较对照组[76.19%(32/42)]高(P<0.05);治疗后观察组VAS评分低于对照组(P<0.05);治疗后观察组肩关节活动范围、日常生活能力评分高于对照组(P<0.05);治疗后观察组血清IL-6、TNF-α、MCP-1水平低于对照组,血清IL-10水平高于对照组(P<0.05)。结论弹拨牵拉胸大肌为主推拿手法联合针刺治疗粘连期肩周炎患者效果确切,可有效减轻炎症反应,缓解肩部疼痛,改善肩关节功能。

Objective To analyze the curative effect of plucking and pulling the pectoralis major muscle as the main massage technique associate with acupuncture in the treatment of patients with periarthritis humeroscapularis at the adhesion stage.Methods A total of 84 patients with periarthritis humeroscapularis at the adhesion stage who were admitted to the Second People’s Hospital of Pingdingshan from March 2019 to October 2020 were selected and divided into observation group(42 cases)and control group(42 cases)according to the random number table method.Among them,the control group was given acupuncture treatment,and the observation group was given plucking and pulling the pectoralis major muscle plus acupuncture.The curative effects,visual analogue scale(VAS),Constant-Murley shoulder score and serum inflammatory factors[interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),monocyte chemoattractant protein-1(MCP-1),interleukin-10(IL-10)]levels were compared between the two groups.Results The total effective rate of 95.24%(40/42)of observation group was higher than that of control group with 76.19%(32/42)(P<0.05).The VAS score of observation group after treatment was lower than that of control group(P<0.05).The range of motion of the shoulder joint and the scores of daily living ability of observation group were higher than those of control group(P<0.05).After treatment,the serum IL-6,TNF-α,and MCP-1 levels of observation group were lower than those of control group,and the serum IL-10 levels was higher than that of control group(P<0.05).Conclusion Plucking and pulling the pectoralis major muscle plus acupuncture is effective in treating patients with periarthritis at the adhesion stage,which can effectively reduce inflammation,relieve shoulder pain,and improve shoulder joint function.
