987例成人畸变产物耳声发射听力筛查结果分析 被引量:4

摘要 目的探讨畸变产物耳声发射(DPOAE)在成人体检人群中听力初筛的应用价值。方法对2019年医院入职体检的987例成人行筛查型DPOAE检查,未通过者1个月内复查DPOAE,复查仍未通过者行听力诊断,包括常规频率及扩展高频纯音测听、声导抗测试和诊断型DPOAE测试,分析听力筛查及诊断结果。结果987例中初筛通过960例(97.26%),未通过27例(2.74%);复筛通过2例(7.41%,2/27),未通过25例(92.59%,25/27,单耳17例,双耳8例);17例进行听力诊断性检查,常规频率和扩展高频纯音测听均异常12例(16耳),鼓室导抗图9耳为A型,4耳为As型,3耳为Ad型;纯音测听常规频率异常但扩展高频正常1例(1耳),鼓室导抗图为As型;纯音测听扩展高频异常但常规频率正常2例(3耳),鼓室导抗图分别为A型、Ad型和C型;常规频率及扩展高频纯音测听均正常2例(4耳),鼓室导抗图均为A型。结论筛查型DPOAE可有效筛选出听力损失及潜在听力损失的受试者,在体检人群中作为普遍听力初筛的方法是可行的。 Objective To analyze of 987 adults’distorted product otoacoustic emissions(DPOAE)hearing screening results for the value of DPOAE in hearing examination of health check-up population.Methods The personnel of the hospital’s pre-employment health check-up in 2019 hearing screened through distorted product otoacoustic emissions,and those who failed the screening were re-examined within 1 month.The pure tone audiometry,tympanometry and DPOAE were re-examined.Results A total of 987 people were screened,960(97.26%)persons passed the primary otoacoustic emissions screening.Twenty-seven(2.74%)persons failed the primary otoacoustic emissions screening,among them 2(7.41%)persons passed the reexamination,25(92.59%)persons failed the reexamination,seventeen of the 25 who failed the reexamination performed pure tone audiometry,tympanometry and DPOAE.12 persons(16 ears)had abnormal pure tone audiometry of the conventional frequency and ultrahigh frequency,tympanogram 9 ears were type A,4 ears were type As,3 ears were type Ad.1 person(1 ear)had abnormal pure tone audiometry of the conventional frequency but normal ultrahigh frequency,tympanogram1 ears was type As.2 persons(3 ears)had normal pure tone audiometry of the conventional frequency but abnormal ultrahigh frequency,tympanogram 1 ears was type A,1 ears was type Ad and 1 was type C.2 persons(4 ears)had normal pure tone audiometry of the conventional frequency and ultrahigh frequency,tympanogram4 ears were type A.Conclusion The screening DPOAE is an objective and rapid test,which can effectively detect subjects with hearing loss or potential hearing loss.This proves that the DPOAE can be used as a common preliminary hearing screening method in health check-up population.
