局限性毛囊角化病1例及文献复习

A case of localized keratosis follicularis and literature review

报告1例局限性毛囊角化病。患者女,46岁。左小腿黄褐色丘疹及斑块10年。皮肤科检查:左小腿可见黄褐色直径约3~5 mm的疣状角化性丘疹,部分融合成斑块。皮肤镜检查:可见中央带有白晕的圆形、星形或多边形的大小不一的黄色区域,周围包绕粉红色无结构区域。部分皮损中央可见黄白色假粉刺样结构,其下可见红色凹陷性糜烂面。皮损组织病理检查:角化过度,角化不全,基底层上棘层松解,形成裂隙,可见大量圆体和谷粒,真皮浅层单核细胞浸润。诊断:局限性毛囊角化病。

A case of localized keratosis follicularis is reported.A 46-year-old woman presented with a rash on her left leg for 10 years,which was tawny,3-5 mm in diameter,and lined by keratotic verrucous papules and plaques.Physical examination showed circular,star-shaped or polygonal yellow areas of various sizes with a white halo in the center of each area,and surrounded by a pink unstructured area.In the middle of some lesions,there was a yellow-white false acne-like structure with a red concave erosive surface.Histopathological examination showed hyperkeratosis and parakeratosis of the epidermis,loosening of the spinous layer on the basal layer with fissures,lots of spheres and kernels,and mononuclear cell infiltration in superficial dermis.The diagnosis of localized keratosis follicularis was made.
