1例犬巨食道症并发吸入性肺炎的诊断和治疗

Diagnosis and treatment of a canine megaesophagus complicated with aspiration pneumonia

巨食道症是一种以食管运动障碍、弥漫性食管扩张为特征的综合征,多发于犬。临床常见呕吐、进行性消瘦、咳嗽及口臭等症状,吸入性肺炎为犬巨食道症最常见、最危险的并发症。文章分享1例典型犬巨食道症并发吸入性肺炎的病例,以期为犬巨食道症的诊治及研究提供参考。通过临床检查、血液学检查、影像学检查后确诊为犬巨食道症;采用对因治疗、对症治疗、调整饮食、改善采食方式等手段治疗9 d后痊愈出院。但因宠物主人所用直立饮食凳较小,患犬无法完全站立,造成二次吸入性肺炎。由于犬巨食道症缺少有效的治疗方法,药物或手术恢复犬食道运动能力的效果不佳,因此,已患巨食道症的犬可通过直立饮食的方式减少因呕吐和反流造成的并发症,延长动物寿命。

Megaesophagus is syndromes characterized by esophageal dyskinesia and diffuse esophageal dilatation,which is common in dogs.Vomiting,progressive weight loss,cough and bad breath are common symptoms.Aspiration pneumonia is the most common and dangerous complication of canine megaesophagus.The article shared a typical case of canine megaesophagus complicated with aspiration pneumonia,in order to provide reference for diagnosis,treatment and research of canine megaesophagus.The dog was diagnosed as canine megaesophagus after clinical examination,hematological examination and imaging examination.After 9 d of treatment,the dog was cured and discharged from hospital by the treatment measures include cause treatment,symptomatic treatment and adjustment of diet and improvement of eating posture.However,the pet owner used an improper size upright dining stool,which made the dog unable to stand completely,resulting in secondary inhalation pneumonia.Due to lack of effective treatment,the effect of drugs or surgery to restore the dog's esophageal motility is unsatisfactory.Therefore,dogs suffering from megaesophagus can reduce the complications caused by vomiting and reflux by eating upright,and prolong the life of animals.
