环状管理在糖尿病患者健康教育中的应用及效果

Application and Effect of Circular Management in Health Education for Diabetic Patients

目的研讨环状管理在糖尿病健康教育护理中的应用效果及对患者血糖水平的影响。方法抽选2017年3月—2018年3月本院收治的326例糖尿病患者为研究对象。按照随机分配原则,将其分为对照组与观察组两组(n=163)。两组患者在糖尿病健康教育护理中分别采用常规管理与环状管理。对比两组患者护理后自我管理能力(合理饮食、血糖监测、足部护理、规律运动、科学用药)、血糖指标[空腹血糖(FPG)、糖化血红蛋白(HbAlc)、餐后2 h血糖(2 h-PG)]与并发症(低血糖、足部病变、酮症酸中毒、糖尿病肾病)发生率。结果观察组自我管理行为评分均高于对照组,差异有统计学意义(P<0.05);观察组FPG、HbAlc、2 h-PG分别为(6.57±0.60)mmol/L、(6.23±1.26)%、(10.32±2.01)mmol/L,均分别低于对照组(11.50±3.16)mmol/L、(8.96±1.74)%、(13.10±3.89)mmol/L,差异有统计学意义(P<0.05);观察组并发症发生率为6.75%,显著低于对照组20.86%,差异有统计学意义(P<0.05)。结论在糖尿病健康教育护理中应用环状管理可有效提高患者自我管理能力,并改善其血糖水平,降低并发症发生率,提高其生活质量。故值得临床推广应用。

Objective To investigate the effect of circular management in the diabetes health education and nursing and its effect on patients′blood sugar level.Methods A total of 326 cases of diabetes that were admitted and treated in our hospital from March 2017 to March 2018 were selected.By the principle of random allocation,they were divided into Control Group and Observation Group(n=163).Routine management and circular management were adopted respectively in patients of both groups for the diabetes health education and nursing.The abilities of self-management after nursing(balanced diet,blood glucose monitoring,foot care,regular exercise,scientific medication),blood glucose indicators(fasting blood glucose(FPG),glycosylated hemoglobin(HbAlc),postprandial 2 h blood glucose(2 h-PG))and complications(hypoglycemia,foot lesions,ketoacidosis,diabetic nephropathy)were compared between the two groups.Results The self-management behavior scores of the Observation Group were higher than that of the Control Group,and the differences were significant(P<0.05).The blood glucose indexes of the Observation Group were respectively 6.57±0.60 mmol/L,6.23±1.26%and 10.32±2.01 mmol/L,which were all lower than that of the Control Group(11.50±3.16 mmol/L,8.96±1.74%,13.10±3.89 mmol/L)with significant differences(P<0.05).The incidence of complications of the Observation Group was 6.75%,much lower compared to that of the Control Group(20.86%),and the difference was significant(P<0.05).Conclusion The application of circular management in the diabetic education and nursing can effectively improve the patients′self-management ability,raise their blood sugar level,reduce the incidence of complications and enhance their quality of life.Therefore,the approach is worthy of clinical promotion and application.
