婴儿泪囊炎和细菌性结膜炎病原菌分布及耐药性分析

目的探讨婴儿泪食炎和细菌性结膜炎病原菌的分布和耐药性,为临床诊断和治疗提供参考方法对637例泪t炎和260例细菌性结膜炎的患儿,留取其眼分泌物做细菌培养并做药物敏感试验结果泪囊炎病原菌生长277例,阳性检出率为43.49%;细菌性结膜炎病原菌生长104例,阳性检出率为40.00%。泪囊炎的病原菌主要为:金黄色葡萄球菌、流感嗜血杆菌、卡他莫拉菌、大肠埃希菌、肺炎克雷伯菌;细菌性结膜炎的病原菌主要为:金黄色葡萄球菌、大肠埃希菌,流感嗜血杆菌、铜绿假单胞菌、肺炎克雷伯菌革兰阳性菌较敏感的药物有氣霉素、左氧氟沙星、利福平、庆大霉素;革兰阴性菌较敏感的药物有左氧氟沙星、头孢曲松、庆大霉素:喹诺酮类药物对大多數病原菌敏感,,结论婴儿泪食炎和细菌性结膜炎感染病原菌种类多,应根据耐药性特征选用抗生素治疗,以确保婴幼儿用药合理、安全、有效。

Objective To investig«ite the distribution and drug resistance of pathogens in infantile dacryocystitis and bacteri«il conjunctivitis and to provide reference for clinical diagnosis and treatment.Methods The eye secretions of 637 cases of dacryocystitis and 260 cases of bacterial conjunctivitis were collected for bacterial culture and drug sensitivity test.Results There were 277 cases of dacryocystitis pathogen growth,the positive rate was 43.49%,and 104 cases of bacterial conjunctivitis pathogen growth,the positive rate was 40.00%.The main pathogens of dacryocystitis were Staphylococcus aureus,Haemophilus influenzae,Moraxella cafcurhalis,Escherichia coli and Klebsiella pneumoniae,the main pathogens ofbacteri;il conjunctivitis are Staphylococcus aureus,Escherichia coli,Haemophilus influenzae,Pseudomonas aeruginosa and Klebsiella pneumoniae.The sensitive dnigs of Gmm-positive bacteria were chloramphenicol,levofloxacin,rifanipicin and gentamicin,while those of Gram-negative bacteria were levofloxacin,ceftriaxone and gentamicin.Quinolones were sensitive to most pathogens.Conclusion There are many kinds of pathogens in infiintile dacryocystitis and bacterial conjunctiviris.Antibiotics should be selected according to the characteristics of drug resistance,so as to ensure the reasonable,safe and effective use of drugs in infants.
