自控式连续硬膜外麻醉镇痛联合缩宫素对阴道分娩产妇的影响

Effect of Self-controlled Continuous Epidural Analgesia Combined with Oxytocin on the Parturient During Vaginal Delivery

目的探讨孕妇实施自控式连续硬膜外麻醉镇痛联合缩宫素对阴道分娩产妇的影响。方法随机抽取2020-01/2021-04月在作者医院分娩的100例无产科并发症及麻醉禁忌证的单胎初产孕妇作为研究对象,按产妇意愿分为研究组和对照组,各50例。研究组采用自控式连续硬膜外麻醉镇痛联合缩宫素进行阴道分娩,对照组不使用任何镇痛药物,观察对比两组分娩情况。结果研究组在分娩镇痛效果,缩短第一、第二产程,自然分娩率方面优于对照组,两组比较差异有统计学意义(P均<0.05),两组第三产程,产后出血量及新生儿评分比较无统计学差异(P>0.05)。结论对孕妇实施自控式连续硬膜外麻醉镇痛联合静滴缩宫素进行阴道分娩,可明显减轻分娩疼痛,缩短第一、第二产程,降低剖宫产率,且对第三产程及母婴安全无影响。

Objective To investigate the effect of self-controlled continuous epidural analgesia combined with oxytocin on vaginal delivery of pregnant women.Methods A random sample of 100 primiparous singleton pregnant women without obstetric complications and contraindications for anesthesia who gave birth in author’s hospital from January 2020 to April 2021 were randomly selected as the research objects.They were divided into study group and control group according to the puerperas wishes,with 50 cases in each group.The self-controlled continuous epidural analgesia combined with oxytocin for vaginal delivery was used by study group and no analgesics was used by the control group.The delivery of two groups were observed and compared.Results The study group was better than the control group in terms of analgesic effect of labor,shortening the first and second stages of labor,and the rate of natural delivery.The difference between the two groups was statistically significant(P<0.05).There were no significant differences in the third stage of labor,bleeding volume and neonatal scores(P>0.05).Conclusion The implementation of self-controlled continuous epidural analgesia combined with intravenous oxytocin for vaginal delivery for pregnant women can significantly reduce labor pain,shorten the first and second stages of labor,reduce the rate of cesarean section,and have no impact on the third stage of labor and the safety of puerperas and infant.
