子宫内膜癌患者血清内脂素水平与病理特征及预后的关系

Relationship between serum visfatin and clinicopathological features and prognosis of endometrial carcinoma

目的探讨子宫内膜癌(EC)患者血清内脂素水平与临床病理特征及预后的关系。方法筛选并收集65例EC患者(EC组)外周血血清,以30例健康体检患者外周血血清作为对照,采用ELISA法检测其外周血清内脂素水平,并根据中位值将EC患者分为低内脂素组和高内脂素组,比较2组患者的临床病理特征、术后短期恢复及长期复发/转移、生存期的差异,并利用单因素及多因素Cox回归分析EC患者术后生存期的影响因素。结果EC组患者血清内脂素水平明显高于对照组(P<0.05);高内脂素组和低内脂素组的肌层浸润深度及淋巴结转移存在统计学差异(P<0.05);高内脂素组患者术后并发症发生率及复发/转移率明显高于低内脂素组患者(P<0.05);高内脂素组患者的术后总生存期明显低于低内脂素组(P<0.05);Cox回归分析发现,内脂素是EC患者术后生存期的独立影响因素(P<0.05)。结论内脂素在EC患者血清中高表达,并与患者预后不良密切相关,可作为EC患者预后评估指标。

Objective To investigate the relationship between serum visfatin level and clinicopathological features and prognosis of endometrial carcinoma(EC)patients.Methods 65 EC patients were selected and collected(EC group),and 30 healthy controls were collected as control,ELISA method was used to detect the serum visfatin level in peripheral serum,and the EC patients were divided into the low visfatin group and high visfatin group according to the median value,the clinical pathological characteristics,the recovery of short-term and short-term postoperative recovery,long-term recurrence/metastasis and survival time were compared in the two groups.Univariate and multivariate Cox regression analysis were used to analyze the influencing factors of postoperative survival in EC patients.Results Serum visfatin level in EC group was significantly higher than that in control group(P<0.05).There were significant differences in the depth of muscular invasion and lymph node metastasis between the high visfatin group and the low visfatin group(P<0.05).The incidence of postoperative complications and recurrence/metastasis rate in high visfatin group were significantly higher than those in low visfatin group(P<0.05).The postoperative overall survival time of high visfatin group was significantly lower than that of low visfatin group(P<0.05).Cox regression analysis showed that visfatin was an independent factor affecting the postoperative survival of EC patients(P<0.05).Conclusion The high serum visfatin of EC patients is closely related to poor prognosis of patients,and can be used as a prognostic indicator of EC patients.
