摘要
目的探讨慢性阻塞性肺疾病(Chronic obstructive pulmonary disease,COPD)合并慢性病贫血(Anemia of chronic disease,ACD)患者全身炎症指标变化情况。方法采用前瞻性病例对照研究,选取2019年3月~2021月3月我院60例无贫血的稳定期COPD患者作为对照组,60例稳定期COPD合并ACD患者作为观察组。两组患者均评价外周血清中白细胞介素-1(IL-1)、白细胞介素-6(IL-6)、白细胞介素-10(IL-10)、肿瘤坏死因子α(TNF-α)、γ干扰素(IFN-γ)、C-反应蛋白(CRP)等炎症指标和促红细胞生成素(EPO)的水平。结果观察组外周血清中IL-1、IL-6、IL-10、TNF-α、IFN-γ、CRP等炎症指标和EPO水平显著高于对照组,差异有统计学意义(P<0.05)。IL-1、IL-6、IL-10、TNF-α、IFN-γ、CRP等炎症指标及EPO与COPD合并ACD发生均呈正相关(r值分别为2.605、3.287、1.905、2.672、3.027、1.683、3.129,P<0.05)。结论炎症介质水平与COPD合并ACD的发生呈正相关,炎症指标是诊断COPD合并ACD的重要标志物。
Objective To investigate the changes of systemic inflammatory markers in patients with chronic obstructive pulmonary disease(COPD)combined with anemia of chronic disease(ACD).Methods From Mar 2019 to Mar 2021,60 patients with COPD without anemia were selected as control group and 60 patients with COPD combined with ACD were selected as observation group.The levels of interleukin-1(IL-1),interleukin-6(IL-6),interleukin-10(IL-10),tumor necrosis factor-α(TNF-α),γ-interferon(IFN-γ),C-reactive protein(CRP)and erythropoietin(EPO)were measured in peripheral serum in both groups.Results The levels of IL-1,IL-6,IL-10,TNF-α,IFN-γ,CRP and EPO in observation group were significantly higher than those in control group(P<0.05).IL-1,IL-6,IL-10,TNF-α,IFN-γ,CRP and EPO were positively correlated with the occurrence of COPD combined with ACD(the r values were 2.605,3.287,1.905,2.672,3.027,1.683 and 3.129,respectively,P<0.05).Conclusion There is significantly positive correlation between the levels of inflammatory mediators and the occurrence of COPD combined with ACD,and inflammatory mediators are important markers for the diagnosis of COPD combined with ACD.
