小针刀配合臂丛神经阻滞麻醉下手法松解术治疗肩周炎临床研究

A study on treating frozen shoulder by small needle knife plus manipulative disengage after brachial plexus block anesthesia

目的:探讨小针刀联合臂丛神经阻滞手法松解术应用于肩周炎治疗中的临床疗效。方法:选取2017年7月-2019年7月期间的80例肩周炎患者,根据数字表法随机将其分为试验组和对照组,均为40例,对照组采用针刺联合手法松解术进行治疗,试验组患者采用小针刀配合臂丛神经阻滞进行治疗。对比观察两组中医症状评分、肩关节功能评分及视觉模拟评分法评分的差异,并检测两组炎性因子水平的差异。结果:干预后,试验组患者视觉模拟评分法评分显著低于对照组,试验组肩关节运动功能评分明显高于对照组,差异显著,P<0.05;试验组治疗后患者白细胞介素-6、C反应蛋白、肿瘤坏死因子α水平显著低于对照组,差异显著,P<0.05;干预后,试验组的中医症状积分显著低于对照组,差异具有统计学意义(P<0.05)。结论:在肩周炎患者中应用小针刀联合臂丛神经阻滞麻醉手法松解术治疗的效果确切,能够缓解患者疼痛,有效改善患者的肩关节运动功能,改善机体的炎症状态,值得临床推广使用。

Objective:To investigate the clinical effect of small needle knife plus manipulative disengage after brachial plexus block anesthesia on frozen shoulder.Methods:80 patients with frozen shoulder from July 2017 to July 2019 were selected and randomly divided into the experimental group and the control group according to the number table method,both of which were 40 cases.The control group was relieved by acupuncture with manipulation.Patients in the experimental group were treated by small needle knife and brachial plexus block.The differences in TCM symptom scores,shoulder joint function scores and visual analogue scores were compared between the two groups,and the differences in the levels of inflammatory factors between the two groups were detected.Results:After the intervention,the visual analogue score in the experimental group was significantly lower than that in the control group,and the shoulder motor function score in the experimental group was significantly higher.The difference was significant,P<0.05.The levels of IL-6,C-reactive protein and tumor necrosis factor-αwere significantly lower,and the difference was significant,P<0.05.After intervention,the TCM symptom score in the experimental group was significantly lower,and the difference was statistically significant(P<0.05).Conclusion:In patients with frozen shoulder,applying small needle knife plus manipulative disengage after brachial plexus block anesthesia has a definite effect,can relieve the pain of the patients,effectively improve the patient’s shoulder joint motor function,ameliorate the body’s inflammatory state,and is worthy of clinical practice.
