对比用米安色林与米氮平治疗抑郁症的效果及对患者认知功能、脑动脉血流速度的影响

To compare the effects of mianserin and mirtazapine in the treatment of d epression and their effects on cognitive function and cerebral arterial blood flow velocity

目的:比较用米安色林与米氮平治疗抑郁症(MDD)的效果及对患者认知功能、脑动脉血流速度的影响。方法:选取114例MDD患者为研究对象。将其随机分为A组与B组。为A组、B组患者分别采用米安色林、米氮平进行治疗。然后比较两组患者汉密顿抑郁量表(HAMD)的评分、简易智力状态检查量表(MMSE)的评分、大脑中动脉(MCA)、大脑前动脉(ACA)、大脑后动脉(PCA)的收缩期血流速度(Vs)、血清二肽基肽酶-4(DPP-4)、5-羟色胺(5-HT)、多巴胺(DA)的水平和不良反应的发生情况。结果:治疗8周后,A组患者的HAMD评分、MCA、ACA、PCA的Vs、血清DPP-4、5-HT的水平均低于B组患者,P<0.05;A组患者的MMSE评分、血清DA的水平均高于B组患者,P<0.05。治疗期间,两组患者不良反应的发生率相比,P>0.05。结论:与采用米氮平相比,用米安色林治疗MDD的效果较好,可显著缓解患者的病情,改善其认知功能,降低其脑动脉血流速度及血清DPP-4的水平,提高其血清5-HT、DA的水平,且安全性较高。

Objective:To compare the effects of mianserin and mirtazapine in the treatment of depression(MDD)and their effects on cognitive function and cerebral artery blood flow velocity.Methods:114 MDD patients were selected as the research object.They were randomly divided into group A and group B.Patients in group A and B were treated with mianserin and mirtazapine,respectively.Then,the scores of HAMD,MMSE,THE systolic blood flow velocity(Vs)of middle cerebral artery(MCA),anterior cerebral artery(ACA)and posterior cerebral artery(PCA),serum dipeptidyl peptidase-4(DPP-4),5-hydroxytryptamine(5-HT)and more in 2 groups were compared The level of paramine(DA)and the occurrence of adverse reactions.Results:After 8 weeks of treatment,THE HAMD score,MCA,ACA,PCA Vs and serum DPP-4,5-HT levels in group A were lower than those in group B,P<0.05;MMSE score and serum DA level in group A were higher than those in group B,P<0.05.During treatment,the incidence of adverse reactions in 2 groups was compared,P>0.05.Conclusion:Compared with mirtazapine,mianserin has better efficacy in the treatment of MDD,which can significantly relieve the condition of patients,improve their cognitive function,reduce the cerebral artery blood flow velocity and the level of serum DPP-4,and improve the level of serum 5-HT and D A,with high safety.
