腹腔镜术后联合药物治疗子宫内膜异位囊肿的临床观察

Clinical observation of laparoscopic surgery combined with drugs in the treatment of endometrial cysts

目的探讨腹腔镜术后联合药物治疗子宫内膜异位囊肿的临床疗效。方法选取2015年1月至2019年12月本院收治的子宫内膜异位囊肿患者67例作为研究对象,采用随机数字表法分为观察组(n=35)与对照组(n=32)。对照组腹腔镜术后给予孕三烯酮治疗,观察组腹腔镜术后给予促性腺激素释放激素激动剂(GnRH-α)治疗。比较两组临床疗效、相关激素水平、不良反应发生率及远期预后。结果观察组治疗总有效率为94.28%,高于对照组的65.62%(P<0.05)。治疗后,两组性激素水平均低于治疗前,且观察组低于对照组(P<0.05)。观察组不良反应发生率为11.42%,低于对照组的40.62%(P<0.05)。观察组痛经率低于对照组,妊娠率高于对照组(P<0.05);两组复发率比较差异无统计学意义。结论子宫内膜异位囊肿腹腔镜术后联合药物治疗效果显著,能有效降低相关激素水平,改善卵巢储备功能,提高妊娠率,值得临床推广应用。

Objective To explore the clinical efficacy of laparoscopic surgery combined with drugs in the treatment of endometriotic cysts.Methods 67 patients with endometriotic cysts admitted to our hospital from January 2015 to December 2019 were selected as the research subject,and they were divided into observation group(n=35) and control group(n=32) according to random number table method. The control group was treated with gestrinone after laparoscopic surgery, and the observation group was treated with gonadotropin releasing hormone agonist(GnRH-α) after laparoscopic surgery. The clinical efficacy, related hormone levels, incidence of adverse reactions and long-term prognosis were compared between the two groups. Results The total effective rate of treatment in the observation group was 94.28%, which was higher than 65.62% in the control group(P<0.05). After treatment, the levels of sex hormones in the two groups were lower than before treatment, and the observation group was lower than the control group(P<0.05). The incidence of adverse reactions in the observation group was 11.42%, which was lower than 40.62% in the control group(P<0.05). The rate of dysmenorrhea in the observation group was lower than that of the control group, and the pregnancy rate was higher than that of the control group(P<0.05);there was no statistically significant difference in the recurrence rate between the two groups. Conclusion The combined treatment of endometriotic cysts after laparoscopic surgery has significant effect, which can effectively reduce the level of related hormones, improve ovarian reserve, and increase pregnancy rate. It is worthy of clinical application.
