关节镜下修补术与开放式肩袖修补术对肩袖损伤患者的临床疗效差异探析

Analysis of the difference in clinical efficacy between arthroscopic repair and open rotator cuff repair in patients with rotator cuff injury

目的探究肩袖损伤患者应用关节镜下修补术、开放式肩袖修补术方案的疗效。方法选取2015年1月至2019年1月本院收治的肩袖损伤需外科手段治疗患者60例为研究对象,按照随机数字表法分为A组与B组,每组30例。A组采用开放式肩袖修补术,B组采用关节镜下修补术。比较两组肩关节活动度、肌力分级、临床指标。结果术后,两组外展、外旋、内旋活动度均明显大于术前(P<0.05),但组间比较差异无统计学意义。术后,两组外展评分、外旋评分、内旋评分均明显高于术前(P<0.05),但组间比较差异无统计学意义。术后,两组ASES评分、UCLC评分均高于术前,VAS评分均低于术前,且B组ASES评分、UCLC评分均高于A组,VAS评分均低于A组,差异有统计学意义(P<0.05)。结论对肩袖损伤患者应用关节镜下修补术或开放式肩袖修补术治疗效果相当,但关节镜下修补术具有微创、术后康复快及疗效更佳的优势,值得临床推广应用。

Objective To analyze the difference in curative effect of arthroscopic repair and open rotator cuff repair in patients with rotator cuff injury.Methods 60 patients with rotator cuff injury who needed surgical treatment in our hospital from January 2015 to January 2019 were selected and they were divided into group A and group B according to the random number table method,with 30 cases in each group.Group A was treated with open rotator cuff repair,and group B was treated with arthroscopic repair.The shoulder joint activity,muscle strength classification and clinical indexes between the two groups were compared.Results After operation,the abduction,external rotation and internal rotation of the two groups were significantly higher than those before operation(P<0.05),but there was no significant difference between the two groups.After operation,the abduction score,external rotation score and internal rotation score of the two groups were significantly higher than those before operation(P<0.05),but there was no significant difference between the two groups.After operation,the ASES score and UCLC score of the two groups were higher than those before operation,and the VAS score was lower than that before operation,the ASES score and UCLC score of group B were higher than A,and the VAS score was lower than A,the difference was statistically significant(P<0.05).Conclusion The treatment effect of arthroscopic repair or open rotator cuff repair for patients with rotator cuff injury is equivalent,but arthroscopic repair has the advantages of minimally invasive,faster postoperative recovery and better curative effect,and it is worthy of clinical application.
