摘要
目的研究子痫前期(PE)患者胎盘微小RNA-199b(miR-199b)、Klotho表达情况,并分析二者与低出生体重的相关性。方法回顾性选取2019年1月至2020年12月在宁德师范学院附属宁德市医院行产前检查和住院分娩的122例中重度PE患者为研究对象,按照发病时的孕周,将PE患者分为早发型PE组71例(发病时妊娠<34周)和晚发型PE组51例(发病时妊娠≥34周);同期选择分娩的健康孕妇80例为对照组。采用实时荧光定量PCR法检测胎盘中miR-199b、Klotho mRNA的水平;采用Pearson法分析miR-199b、Klotho mRNA水平与新生儿体重的相关性。结果与对照组相比,早发型PE组、晚发型PE组舒张压、收缩压较高,新生儿出生体重、分娩孕周较低,胎盘中miR-199b水平、Klotho mRNA水平较低,差异均有统计学意义(P<0.05);与晚发型PE组相比,早发型PE组舒张压、收缩压较高,新生儿出生体重、分娩孕周较低,胎盘中miR-199b水平、Klotho mRNA水平较低,差异均有统计学意义(P<0.05)。PE患者胎盘miR-199b水平、Klotho mRNA水平与新生儿体重呈正相关(r=0.594、0.669,P<0.05)。二元Logistic回归分析,miR-199b低表达、Klotho低表达是PE患者低出生体重儿的独立危险因素(OR=2.149,95%CI:1.651~2.798;OR=2.151,95%CI:1.527~3.029,P<0.05)。结论miR-199b、Klotho在PE患者胎盘组织中呈低表达,miR-199b低表达、Klotho低表达是影响PE患者低出生体重儿的独立危险因素。
Objective To study the expression of microRNA-199b(miR-199b)and Klotho in the placenta of patients with preeclampsia(PE),and to analyze the correlation between the two and low birth weight.Methods A total of 122 patients with moderate to severe PE who underwent prenatal checkups and hospital delivery in Ningde Hospital Affiliated to Ningde Normal University from January 2019 to December 2020 were selected as the research objects.According to the gestational week at the time of onset,the PE patients were divided into the early-onset PE group of 71 cases(Pregnancy<34 weeks at the onset)and 51 cases in the late-onset PE group(pregnancy≥34 weeks at the onset);80 healthy pregnant women who gave birth in this hospital at the same time were selected as the control group.Real-time fluorescent quantitative PCR method was used to detect the level of miR-199b and Klotho mRNA in the placenta;Pearson method was used to analyze the correlation between the level of miR-199b and Klotho mRNA and the weight of newborns.Results Compared with the control group,the diastolic pressure and systolic pressure of the early-onset PE group and the late-onset PE group were higher,the birth weight and gestational week of the newborn were lower,the miR-199b level and Klotho mRNA level in placenta were lower,the differences were statistically significant(P<0.05);compared with late-onset PE group,the diastolic blood pressure,systolic blood pressure of early-onset PE group were higher,the birth weight and gestational age were lower,and the levels of miR-199b and Klotho mRNA in placenta were lower,the differences were statistically significant(P<0.05);the placental miR-199b level and Klotho mRNA level of PE patients were positively correlated with newborn weight(r=0.594,0.669;P<0.05).Binary Logistic regression analysis showed that low expression of miR-199b and low expression of Klotho were independent risk factors for low birth weight infants in PE patients(OR=2.149,95%CI:1.651~2.798,OR=2.151,95%CI:1.527~3.029,P<0.05).Conclusion The expression of miR-199b and Klotho is low in the placenta of PE patients,and the low expression of miR-199b and Klotho are independent risk factors affecting low birth weight infants in PE patients.
