摘要
目的:探讨胰腺神经内分泌肿瘤不同病理分级患者的磁共振成像(MRI)特征。方法:回顾性分析2016年至2020年本院收治的85例胰腺神经内分泌肿瘤患者的临床资料,统计肿瘤病理分级情况,比较不同病理分级患者的一般资料和其病灶的CT、MRI的影像学特征。结果:G1、G2、G3级患者的基本资料比较,差异无统计学意义(P>0.05);不同病理分级患者肿瘤形态、胰管扩张和肿瘤转移等MRI影像学特征比较,差异均有统计学意义(P<0.05),分级越高肿瘤组织形态越趋于不规则,胰管扩张与转移风险随之增加;不同病理分级动脉期低/等/高强化、门脉期低/等/高强化等信号特点比较,差异均有统计学意义(P<0.05)。结论:胰腺神经内分泌肿瘤不同病理分级患者分级越高MRI成像肿瘤组织形态越趋于不规则,胰管扩张与转移风险随之增加;不同病理分级患者的动脉期低/等/高强化、门脉期低/等/高强化等信号特征存在差异。
Objective:To explore magnetic resonance imaging(MRI)features of patients with different pathological grades of pancreatic neuroendocrine tumors.Methods:A retrospective analysis of the clinical data of 85 patients with pancreatic neuroendocrine tumors admitted to our hospital from 2017 to 2019 was performed.The tumor pathological grade was statistically analyzed.Then,the general data of patients with different pathological grades and the imaging features of CT and MRI of their lesions were compared.Results:There were no statistically significant difference in the general data of the patients with grades G1,G2,and G3(P>0.05).However,there were statistical differences in the MRI imaging features such as tumor morphology,pancreatic duct dilatation and tumor metastasis among the patients with different pathological grades(P<0.05).The higher the grade was,the more irregular the tumor tissue morphology was,and the risk of pancreatic duct dilation and metastasis was increased.Further,there were statistically significant differences in signal characteristics of low/equal/high enhancement in arterial phase and low/equal/high enhancement in portal vein phase among the patients with different pathological grades(P<0.05).Conclusions:The higher the grade is,the more irregular the tumor tissue morphology in MRI imaging is,and the risk of pancreatic duct dilation and metastasis is increased.Further,there are statistically significant differences in signal characteristics of low/equal/high enhancement in arterial phase and low/equal/high enhancement in portal vein phase among the patients with different pathological grades.
