摘要
目的探究多囊卵巢综合征患者性激素水平变化及其与胰岛素抵抗的关系。方法选择2017年3月—2020年2月于本院进行诊治的114例多囊卵巢综合征患者作为研究组,另选取同期于本院进行健康体检的自愿者114人作为对照组,采集所有参与者空腹静脉血,检测血液标本中各性激素水平。比较2组性激素水平[雌二醇(E2)、促卵泡生成素(FSH)、促黄体生成素(LH)、睾酮(T)水平]及胰岛素抵抗指数(HOMA-IR),另分析研究组患者性激素水平变化与HOMA-IR的相关性。结果研究组LH水平为(20.53±6.37)pg/mL、T水平为(102.39±38.75)ng/dL、LH/FSH为(2.61±0.63)、HOMA-IR为(8.36±2.21),均高于对照组的(13.39±5.12)pg/mL、(48.28±10.36)ng/dL、(1.24±0.31)、(2.28±0.59);研究组E2水平为(43.79±16.58)pg/mL、FSH水平为(8.31±2.10)pg/mL,均低于对照组的(62.47±17.35)pg/mL、(11.29±4.24)pg/mL,差异均有统计学意义(P<0.01);E2水平与HOMA-IR呈负相关(P<0.01),T及LH与HOMA-IR呈正相关(P<0.01)。结论性激素水平与胰岛素抵抗间存在相关性,参与了多囊卵巢综合征的发生及发展,在临床上需给予足够重视,以改善性激素水平与胰岛素抵抗的局面。
ObjectiveTo explore the relationship between the changes of sex hormone levels and insulin resistance in patients with polycystic ovary syndrome(PCOS).MethodsA total of 114 patients with PCOS who were diagnosed and treated in our hospital from March 2017 to February, 2020 were selected as a research group, and 114 volunteers who had physical examination in our hospital during the same period were selected as a control group. Fasting venous blood samples were collected from all the participants to detect the levels of sex hormones in blood samples. The levels of sex hormones[estradiol(E2), follicle stimulating hormone(FSH), luteinizing hormone(LH), testosterone(T)] and insulin resistance index(HOMA-IR) were compared between the two groups, and the correlations between the changes of sex hormone levels and HOMA-IR were analyzed.ResultsThe levels of LH, T, LH/FSH and HOMA-IR in the research group were(20.53 ± 6.37)pg/mL,(102.39 ± 38.75) ng/dL,(2.61 ± 0.63) and(8.36 ± 2.21) respectively, which were higher than those in the control group [(13.39 ± 5.12) pg/mL,(48.28 ± 10.36) ng/dL,(1.24 ± 0.31) and(2.28 ± 0.59) respectively];the levels of E2 and FSH in the research group were(43.79 ± 16.58) pg/mL and(8.31 ± 2.10) pg/mL respectively, which were lower than those in the control group [(62.47 ± 17.35)pg/mL and(11.29±4.24) pg/mL respectively], and there were significances between the two groups(all P < 0.05). The level of E2 was negatively correlated with HOMA-IR(P < 0.01), while the levels of T and LH were positively correlated with HOMA-IR(P < 0.01).ConclusionThere is a correlation between sex hormone level and insulin resistance, which is involved in the occurrence and development of PCOS. Therefore, in the clinic, we should pay enough attention to improve the situation of the sex hormone levels and insulin resistance.
