髋关节超声检查格拉夫法在婴幼儿发育性髋关节异常筛查中的价值

Value of the Hip Ultrasound Inspection Graff Method in Infants Developmental Hip Abnormal Screening

目的探讨髋关节超声检查格拉夫(Graf)法在婴幼儿发育性髋关节异常筛查中的价值。方法选择我院2017年1月至2019年6月肉眼不能发现畸形的婴幼儿作为研究对象,共1550例,3100个髋关节接受检查,采用佳能Aplio 500超声诊断仪对研究对象进行髋关节超声检查,比较正常髋关节和异常髋关节的α角和β角,分析超声诊断婴幼儿髋关节异常的结果。结果与正常髋关节对比,异常髋关节的α角明显更小,β角明显更大,P<0.05。共3100个髋关节,其中正常髋关节2790个(90.00%),异常髋关节310个(10.00%)。在异常髋关节中,发育性髋关节异常共20个(0.65%)。超声指标α角和β角的统计结果如下:α角42.00°~78.55°,平均为65.83°±3.69°;β角42.51°±3.51°。结论随着检查技术的不断改进,发育性髋关节异常的早期诊断准确率得到了明显的提高,早期治疗方案也获得了更大程度的完善。髋关节超声检查Graf法适用于6个月以内的婴幼儿发育性髋关节异常筛查,准确度较高,具备无创性、可重复操作的优点。

Objective To investigate the role of Graf in screening developmental hip dysplasia(DDH)in infants.Methods From January 2017 to June 2019,a total of 1,550 infants and 3,100 hips in our hospital were selected as the subjects for the study,and the subjects were examined with Canon Aplio 500 ultrasonic diagnostic instrument for the examination of the hips,and the alpha Angle and beta Angle of the normal and abnormal hips were compared.The results of ultrasonic diagnosis of infant hip joint abnormality were analyzed.Results Compared with normal hip joint,the alpha Angle of abnormal hip joint was significantly smaller and the beta Angle was significantly larger,P<0.05.There were 3,100 hips,including 2,790 normal hips(90.00%)and 310 abnormal hips(10.00%).Among the abnormal hips,20(0.65%)were developmental.The statistical results of alpha Angle and beta Angle were as follows:alpha Angle 42.00°-78.55°,with an average of 65.83°±3.69°,Beta Angle 42.51°±3.51°.Conclusion With the continuous improvement of examination technology,the early diagnosis accuracy of developmental hip joint abnormalities has been significantly improved,and the early treatment program has also been improved.The Graf method for hip ultrasound examination is applicable to the screening of DDH in infants within 6 months,with high accuracy and the advantages of non-invasive and repeatable operation.
