高龄孕产妇与适龄孕产妇围生期血液学检验指标变化及妊娠结局比较

Changes of Hematologic Test Indexes and Pregnancy in Elderly and Age-appropriate Maternal Perinatal Period and Outcome Comparison

目的比较高龄孕产妇与适龄孕产妇围生期血液学检验指标的变化及妊娠结局。方法挑选在本院产检的高龄孕产妇94例为试验组,适龄孕产妇94例为对照组,比较两组孕产妇围生期的各项凝血功能指标、各项血脂指标及妊娠结局。结果孕30周、38周和产后3 d两组孕产妇的血浆凝血酶时间(TT)、凝血酶原时间(PT)、D-二聚体(D-D)比较,差异无统计学意义(P>0.05)。试验组孕30周、38周和产后3 d的三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、总胆汁酸(TBA)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)均高于对照组(P<0.05)。对照组发生胎儿窘迫1例、产后出血2例、窒息0例、早产1例,妊娠不良结局总发生率4.26%(4/94);试验组发生胎儿窘迫2例、产后出血4例、窒息1例、早产5例,妊娠不良结局总发生率12.77%(12/94),组间比较差异有统计学意义(P<0.05)。结论与适龄孕产妇相比,处于妊娠期的高龄孕产妇具有更高的妊娠不良结局风险,临床要密切监测高龄孕产妇的各项凝血功能指标及血脂指标,以及时发现异常情况,并有效改善妊娠结局。

Objective To compare the changes of blood test indexes and pregnancy outcomes between elderly and age-appropriate maternal perinatal period.Methods A total of 94 elderly pregnant women were selected as the experimental group and 94 aged pregnant women as the control group in the hospital.The coagulation function indexes,lipid indexes and pregnancy outcomes of the two groups were compared and analyzed.Results There was no significant difference in plasma thrombin time(TT),prothrombin time(PT)and D-dimer(DD)between the two groups of pregnant women at 30 weeks,38 weeks and 3 days postpartum(P>0.05).Triglycerides(TG),low-density lipoprotein cholesterol(LDL-C),total bile acid(TBA),total cholesterol(TC),high-density lipoprotein cholesterol(HDL-C)were higher than the control group(P<0.05).In the control group,fetal distress occurred in 1 case,postpartum hemorrhage in 2 cases,asphyxia in 0 cases,premature birth in 1 case,and the total incidence of pregnancy outcomes was 4.26%(4/94).There were 2 cases of fetal distress,4 cases of postpartum hemorrhage,1 case of asphyxia,and 5 cases of premature birth in the experimental group.The total incidence of pregnancy outcomes was 12.77%(12/94),and the group was comparable(P<0.05).Conclusion Compared with age-appropriate pregnant women,the elderly pregnant women have a greater risk of pregnancy,so clinical monitoring of the elderly pregnant women’s coagulation function indicators and blood lipid indicators,so that they can immediately find abnormal conditions and effectively improve the outcome of pregnancy.
