摘要
目的:探讨怀孕早期维生素D水平及相关影响因素。方法:选取北京儿童医院顺义妇儿医院建档的387名孕早期妇女,由统一培训的专业人员收集其基本信息。早上空复采血后,检测维生素D浓度并收集维生素D代谢相关基因位点的SNPs数据。结果:孕妇维生素D总体水平为(20.13±9.11)ng/mL,维生素D不足率为91.2%。维生素D浓度与年龄、孕前BMI、孕次、产次、GCrs7041多态性等相关性无统计学差异(P>0.05);与GCrs2282679多态性相关(P<0.05),与T/T型相比,G/G型维生素D浓度低3.46个单位(95%CI-6.53~-0.39,P=0.027),G/T型维生素D浓度低1.75个单位(95%CI-3.65~0.16,P=0.073)。Logistic回归分析显示,GCrs2282679的G/G型具有较高的维生素D缺乏程度(OR=0.273,95%CI 0.126~0.592,P=0.001),而高年龄人群具有较低的维生素D缺乏程度(OR=2.679,95%CI 1.040~6.902,P=0.041)。结论:怀孕早期孕妇维生素D普遍不足,且维生素D浓度与GCrs2282679的多态性有关,有必要开展进一步研究。
【Objective】To investigate the relationship between vitamin D levels and related factors among early pregnant women.【Method】Totally 387 women in early pregnancy registered in Shunyi Women and Children’s Hospital of Beijing Children’s Hospital were selected.Basic information was collected by trained professionals.After fasting blood collection in the morning,vitamin D concentration and vitamin D metabolism-related gene locus were detected for analysis.【Result】The overall vitamin D concentration of pregnant women was(20.13±9.11)ng/mL,and the vitamin D deficiency rate was 91.2%.There were no statistically differences between vitamin D concentration and age,pre-pregnancy BMI,pregnant times,parity,or GCrs7041 polymorphism(P>0.05).The vitamin D concentration was associated with GCrs2282679 polymorphism(P<0.05).Compared with T/T type,the vitamin D concentration of GG type was lower with3.46 units(95%CI-6.53~-0.39,P=0.027)and the vitamin D concentration of G/T type was lower with 1.75 units(95%CI-3.65~0.16,P=0.073).Logistic regression analysis showed that the G/G type of GCrs2282679 had a higher degree of vitamin D deficiency(OR=0.273,95%CI0.126~0.592,P=0.001),while the elderly population had a lower degree of vitamin D deficiency(OR=2.679,95%CI1.040~6.902,P=0.041).【Conclusion】Vitamin D deficiency of pregnant women is common in early pregnancy.Vitamin D level is related to the polymorphism of GCrs2282679,which need further research.
