外伤性睫状体脱离的临床治疗观察

Clinical Observation of Traumatic Ciliary Body Detachment

目的:以期通过对外伤性睫状体脱离患者的临床表现与治疗研究进行探讨.方法:回顾分析了我院2017年6月-2020年6月期间收治的眼部外伤导致的外伤性睫状体脱离患者28例28眼,行超声生物显微镜(UBM)等检查确定了睫状体脱离的部位和范围,给与了药物治疗,药物保守治疗效果不明显者行手术治疗.结果:7例患者通过保守医学药物治疗得到了治愈,19例患者通过手术治疗1次成功,2例患者2次治疗成功;出院后随访2-6个月,UBM显示所有患者眼部睫状体成功的复位;患者的视力、眼压、前房深度以及视网膜的水肿程度等得到明显改善.结论:睫状体脱离在眼部外伤后多见,常伴有持续性的低眼压及严重视力损害,UBM的详细检查对于睫状体脱离的早期诊断和处理具有重要意义;患者在医院确诊后及时采取药物保守治疗,药物治疗被证实无效者及时采取手术治疗,可有效恢复患者的视功能;睫状体缝合复位术是一种临床可靠有效的治疗方法.

Objective:In order to explore the clinical manifestations and treatment of traumatic cyclodialysis patients.Methods:28 cases(28 eyes)of traumatic ciliary body detachment caused by ocular trauma in our hospital from June 2017 to June 2020 were retrospectively analyzed,Ultrasound biomicroscopy(UBM)was performed to determine the location and extent of ciliary body detachment,Given medication,If the effect of conservative treatment is not obvious,surgical treatment should be performed.Result:Seven patients were cured by conservative medicine,19 patients were successfully treated once by operation and 2 cases twice;The patients were followed up for 2-6 months after discharge.UBM showed that the ciliary body of all patients was successfully reset;The visual acuity,intraocular pressure,anterior chamber depth and retinal edema were significantly improved.Conclusion:Ciliary body detachment is common after ocular trauma,often accompanied by persistent hypotony and severe visual impairment,The detailed examination of UBM is very important for the early diagnosis and treatment of ciliary body detachment;After the diagnosis in the hospital,the patients were treated conservatively with drugs in time.If the drug treatment was proved invalid,the patients were treated with surgery in time,It can effectively restore the visual function of patients;Ciliary body suture reduction is a reliable and effective treatment method.
