从《伤寒论》方证的或然症谈中医误诊

Discussion on the misdiagnosis in TCM from probable syndromes of formula-syndrome in Shanghan Lun

《伤寒论》小青龙汤证、小柴胡汤证、真武汤证、四逆散证及通脉四逆汤证方证中的或然症,对于临证准确辨识相关方证的病位、病机,选择合适治疗药味,减少中医误诊误治,具有重要参考价值。文章试梳理这5个方证的或然症,初步探讨其在减少中医误诊方面的重要作用,并提出辨清病位与病机,选定主方,随症加减用药的诊治思路,作为减少中医误诊发生的有效途径。

The probable syndromes of formula-syndrome in Shanghan Lun such as syndrome could be cured in Xiaoqinglong Decoction,Xiaochaihu Decoction,Zhenwu Decoction,Sini Powder and Tongmai Sini Decoction have great reference value in clinical practice to accurate identifying the disease position and pathogenesis among relative formula-syndromes,it can give the guidance to select the right medicine to reduce the TCM misdiagnosis and mistreatment.In this paper.5 probable syndromes were analyzed and the important functions in reducing TCM misdiagnoses among them were preliminary probed,then the diagnosis and treatment ideas in correct identifying the disease location and pathogenesis were put forward,major formula selection and add or subtract medicine alone with the symptoms changing,thus it could be an effective way to reduce the TCM misdiagnosis.
